It provides national and local catalogues of requestable tests, rules and guidance for electronic requesting (via the Welsh Clinical Portal (WCP) and GP systems) and a web-based knowledge resource for all clinical staff. The introduction of the new national Laboratory Information Management System (LIMS) for Wales means that the operational WPH needs to be reviewed and updated to ensure that it is fit for purpose in the future environment. Any sample booked without an appointment will not be tested. To make an appointment for either NHH please phone 01873 733062 Reference Ranges: It has been decided at an All Wales level that reference ranges would not normally be published in the handbook, but would be produced with reports and pulled from LIMS. This negates the possibility that a clinical decision could be made on an out of date reference range. Please only use sample containers supplied in the pack. These sample containers have been tested to exclude cytotoxicity to sperm and are pre-weighed for the purpose of measuring semen volume. Batch number and weight is marked on the base of the container. Samples in any other container will NOT be accepted. Please read the patient information leaflet thoroughly. Samples should be produced at home and delivered to the Pathology Department for the appropriate appointment time within 60 minutes of production. Facilities for sample production on site at Nevill Hall Hospital are NOT currently available. Please note it is against the Law to produce a semen sample in a public toilet. Request Form All specimens should be accompanied by a completed test request form Please use addressograph labels to fill in patient details, with full name, address with postcode and D.O.B. The sections in red on the request form will be filled out during the appointment. Semen analysis for fertility investigation. The following tests have not been validated and are currently not accredited to ISO15189 standards:-liquefaction, viscosity, agglutination. Andrology Specimen Acceptance Criteria All Andrology specimen containers and request forms should be labelled with a printed addressograph label containing the following essential patient information: Patients full name Date of Birth Patients address, hospital number or NHS number* Destination for the report and the requesting clinician *Printed specimen labels from DPHC Brecon (Dering Lines), Prisons etc. can state the location instead of a hospital / NHS number. The date and time of sample collection will be confirmed by the patient upon receipt of the specimen in the laboratory. Vasectomy clinic only: Specimens requested by the ABUHB vasectomy service should be labelled with a printed addressograph which contains the vasectomy clinic M number, Patients’ name and Date of Birth. Specimens which have leaked in transit or have not met transport requirements (kept close to body temperature or with significant delay in delivery) will also be rejected. Specimens received which do not fulfil the above criteria will not be analysed and a rejection report will be issued accordingly. Post Vasectomy Semen Analysis Post vasectomy samples are accepted by appointment only and must be received in the lab within 1 hour of production in order to test for motility of any sperm present. We would suggest appointments should be booked at least 2-3 weeks in advance, at certain times however the wait for an appointment may be longer. Results will be sent out directly from the Vasectomy Clinic, 2-3 weeks after the post vasectomy semen analysis appointment. Retrograde ejaculation investigation (RE) We prefer to have specimens produced on-site in NHH Andrology, specimens must be received within 1 hour of production by appointment only. Patients will need a 60ml non-toxic container for semen and a 250ml container for urine. Any semen produced after orgasm should be collected, also a post-ejaculatory urine sample. Only types I & III are present in nature & type III series is more important because it includes heme. 1 Heme Biosynthesis Heme biosynthesis occurs through the following steps: 1-The starting reaction is the condensation between succinyl-CoA ((derived from citric acid cycle in the mitochondria)) & glycine, this reaction is a rate limiting reaction in the hepatic heme synthesis, it occurs in the mitochondria & is catalyzed by ALA synthase (Aminolevulinate synthase) enzyme in the presence of pyridoxal phosphate as a cofactor. The product of this reaction is α-amino-β-ketoadipate which is rapidly decarboxylated to form δ-aminolevulinate (ALA). 2-In the cytoplasm condensation reaction between two molecules of ALA is catalyzed by ALA dehydratase enzyme to form two molecules of water & one 2 molecule of porphobilinogen (PBG) which is a precursor of pyrrole.

Sc.) 2000 Universiti Putra Malaysia A thesis submitted for the degree of Doctor of Philosophy at The University of Queensland in 2014 School of Medicine ABSTRACT Bilirubin (BR), a toxic waste product of degraded haem, is a potent antioxidant at physiological concentrations. To achieve the maximum benefit of BR, its intracellular level needs to be carefully regulated. A system comprising of two enzymes, haem oxygenase-1 (HMOX1) and cytochrome P450 2A5 (CYP2A5) exists in the endoplasmic reticulum (ER), responsible for regulating BR homeostasis. This system is induced in response to oxidative stress. In this thesis, oxidative stress caused accumulation of these enzymes in mitochondria — major producers and targets of reactive oxygen species (ROS) — is demonstrated. To understand the significance of this intracellular targeting, properties of microsomal and mitochondrial BR metabolising enzymes were compared and the capacity of mitochondrial CYP2A5 to oxidise BR in response to oxidative stress is reported. Microsomes and mitochondrial fractions were isolated from liver homogenates of DBA/2J mice, administered with sub-toxic dose of pyrazole, an oxidant stressor. The purity of extracted organelles was determined by analysing the expressions and activities of their respective marker enzymes. HMOX1 and CYP2A5 were significantly increased in microsomes and even more so in mitochondria in response to pyrazole-induced oxidative stress. By contrast, the treatment did not increase either microsomes or mitochondrial Uridine-diphosphate-glucuronosyltransferase 1A1 (UGT1A1), the sole enzyme that catalyses BR elimination through glucuronidation.

Bilirubin formation proceeds in several steps: • Removal of iron and globin from the hemoglobin molecule together with opening of the porphyrin ring between pyrrole I and II and release of carbon monoxide gives rise to a linear tetrapyrrole, green pigment biliverdin . • Reduction of the central methenyl bridge between pyrrole III and IV in the biliverdin molecule by action of biliverdin reductase produces yellow bilirubin . This form of bilirubin is denoted as unconjugated bilirubin . It is insoluble in water , and rather easily passes biomembranes and enters the cells, where it is strongly toxic. That is why bilirubin, following release from its site of origin, is transported in circulation bound to albumin .

Ehrlich'' found, in 1901, a pathological urine from which there appeared a strong red tint by the addition of p-dimethylamino- benzaldehyde in an acid medium by hydrochloric acid, and the sub- stance to offer the phenomenon was proved to owe to urobilinogen by 0. Neubauer2' in 1903. The Ehrlich's aldehyde reaction is, accord- ing to H. Fischer, 3' a specific reaction to pyrrole and its derivatives of which a or R position has methenyl group, especially in the former. But urobilinogen has no methenyl, and then, in 1931, H. Fischer and R. Hess4' suggested that urobilinogen will be split into neo(isoneo)- bilirubinic acid at the central methylene bridge by the Ehrlich's aldehyde reagent and that p-dimethylaminobenzaldehyde will be more reactive to the free end methenyl. But H. Fischer and E. Adler, 5' in 1932, denied the Hess's suggestion judging from the fact that a condensation product of 2 moles of neo(isoneo)-xanthobilirubinic acid and 1 mole of aldehyde or aliphatic ketone strongly reacts with the Ehrlich's aldehyde reagent, and they pointed out that the Ehrlich's aldehyde reagent perhaps acts upon three methylene bridges of urobilinogen respectively. According to H. Fischer,s' the condensation product of neobilirubinic acid and p-dimethylaminobenzaldehyde was light yellow needle-like crystal and not identical with the Ehrlich's aldehyde reactant, and its colour remained yellow even when it was dissolved into hydrochloric acid, and its oxidation product by ferric chloride was light pink.
