    GETTING A COURT ORDER FOR NAME CHANGE MIAMI COUNTY What to File: You will need: 1) Completed Application for Change of Name (Adults: Forms 21.0, 21.5, Vital Statistics Informational Form (if born outside of Ohio) (Minors: Forms 21.2, 21.4, 21.5, 21.21, Vital Statistics Informational Form (if born outside of Ohio); 2) Certified copy of birth certificate (Minors); 3) Driver’s license or state identification card (actual ID, not a copy); and 4) $130.00 filing fee. Note: An applicant for change of name must have lived in Miami County for at least a year at the time of the application. The applicant need not present proof of residency. However, the applicant must sign the Application for Change of Name, which includes a statement that the applicant has lived in the county for at least one year immediately prior to filing the application. Note: The Miami County Probate Court does not grant fee waivers for name changes. Equitas Health and TransOhio are currently working to secure financial assistance for those who need help with fees. Blankenship (deceased) and Carolyn J. Blankenship 4035 North State Route 48 Covington, Ohio 45318 Present Occupant: Vacant Significance: The Martin Steinhilber House was built circa 1859. The house is a two-story structure of Vernacular design with Italianate stylistic elements. The structure has a wood frame and brick insulation covered by wood shiplap siding. Handmade decorative brackets ornament the eaves and the front has an Eastlake Detail porch. The adjacent houses are of the same period (circa 1850-1870). Martin Steinhilber was a German immigrant and prominent weaver of carpets and coverlets. Examples of his work are included in the Abby Aldrich Rockefeller Folk Art Collection, Williamsburg, Virginia. Martin Steinhilber bought the property In 1859, and taxes for structures on the property for the year 1860 abruptly increased, from $90 to $159. An earlier abrupt tax increase (from $0 to $90' in 1854) supports an earlier construction date. Steinhilber family records indicate that the family lived on the property as early as 1857, when they may have been renting the structure or an earlier structure on the same site.
