Efforts to promote collaborative primary care and public health initiatives have been underway for $ several years; however, opportunity remains to build capacity at the local level to foster integrated population health improvement. To help facilitate partnership endeavors, the Institute of Medicine (2012) identified $five principles that enable primary care and governmental public health to work together: a shared goal of population health; community engagement; aligned leadership; sustainable systems; shared and collaborative use of data and analysis.
