Published November 2017 3 Montana CAST-S 2017 Editorial and layout support was provided by Peter Christensen, Punchline Communications Inc. 4 Montana CAST-S 2017 About the Authors Scott Poland, EdD Currently a professor at Nova Southeastern University’s College of Psychology, Dr. Scott Poland is also the co- director of the NSU Office of Suicide and Violence Prevention in Fort Lauderdale, Florida. He has worked in schools as a psychologist or a director for twenty-six years and is still very involved in school crisis response and consultation. He has recently provided on-site assistance after suicides in several school districts. As survivors of the suicide of his father, he and his family are examples of those who never believed a suicide could happen. After being promoted to director of psychological services in Cypress-Fairbanks ISD and facing the suicides of several students, Dr. Poland became dedicated to suicide prevention. In 1982, the district superintendent asked him what he was going to do about these suicides. At the time, he answered that he did not know. Figuring out how to prevent youth suicide has been his highest professional priority ever since, and he has presented more than 1,000 times on the topics of school crisis and suicide intervention. Dr. Poland is a licensed psychologist and an internationally recognized expert on youth suicide and school crisis, and he has authored or coauthored five books on the subject. Additionally, with his wife, Donna, he has authored the Suicide Safer Schools Plan for the state of Texas. Dr. Poland is a past president of the National Association of School Psychologists and a past director of the Prevention Division of the American Association of Suicidology. He has testified about the mental health needs of children before the U.S. Congress on four occasions and has personally assisted school communities after sixteen school shootings and after acts of terrorism, natural disasters, and numerous suicide clusters. Donna Poland, PhD Dr. Donna Poland has been a professional educator for thirty-seven years, of which twenty-seven years were in public schools in the great state of Texas where, she says, “everything is BIG!” She remembers her surprise, as a first-year teacher, when she discovered that her middle school classes averaged over thirty students, and she had six classes each day! She very quickly discovered that she needed to get to know her students and build a respectful and nurturing environment before she could even consider teaching the curriculum. Having that vital connection with each student was the reason she walked joyfully into the building every day. After seventeen years in the classroom, Dr. Poland began her administrative journey as a director of instruction, an at-risk coordinator, an associate principal, and, finally, a principal. Immediately upon taking a leadership role, she was faced with the tragedy of a scholar athlete taking his life by suicide just days before he was due to enter ninth grade. Tragically, that was not the only suicide she experienced. Every year resulted in working with parents, staff, and children in the aftermath of a death by suicide or an accidental death. The next ten years (at a small private college-preparatory school in Florida), Dr. Poland continued to need her skills for developing programs, educating staff, and working with parents regarding the signs of depression and suicide.     Some Facts About Suicide and Depression WHAT IS DEPRESSION? Depression is the most prevalent mental health disorder. The lifetime risk for depression is 6 to 25%. According to the National Institute of Mental Health (NIMH), 9.5% or 18.8 million American adults suffer from a depressive illness in any given year. There are two types of depression. In major depression, the symptoms listed below interfere with one’s ability to function in all areas of life (work, family, sleep, etc). In dysthymia, the symptoms are not as severe but still impede one’s ability to function at normal levels. Common symptoms of depression, reoccurring almost every day: o Depressed mood (e.g. feeling sad or empty) o Lack of interest in previously enjoyable activities o Significant weight loss or gain, or decrease or increase in appetite o Insomnia or hypersomnia o Agitation, restlessness, irritability o Fatigue or loss of energy o Feelings of worthlessness, hopelessness, guilt o Inability to think or concentrate, or indecisiveness o Recurrent thoughts of death, recurrent suicidal ideation, suicide attempt or plan for completing suicide A family history of depression (i.e., a parent) increases the chances (by 11 times) than a child will also have depression. The treatment of depression is effective 60 to 80% of the time. However, according the World Health Organization, less than 25% of individuals with depression receive adequate treatment. If left untreated, depression can lead to co-morbid (occurring at the same time) mental disorders such as alcohol and substance abuse, higher rates of recurrent episodes and higher rates of suicide.

We did not substantiate the complainant’s allegations that a change in medication contributed to her husband’s death by suicide, that managers improperly tried to “commit” him to a Veterans Home, or that staff told her she was not her husband’s power of attorney (POA). Medical record documentation reflects that the patient’s medication had not been changed. His chronic depression was attributed to his medical and mental health conditions and to his substantial psychosocial stressors. Further, the medical record does not support the allegations related to the Veterans Home or POA. We found, however, that the Suicide Prevention Coordinator did not participate in the evaluation and ongoing monitoring of the patient, and the treatment team did not complete a suicide risk assessment at the time of the patient’s discharge in February.

Clearly, cognitive processes play a significant role in suicidal thoughts and behaviors, but it is also important to increase the focus on affect. There is support for the role of affect and the fact that cognition and affect combine with one another to impact suicidal behaviors. These findings may be advanced through the application of a theoretical model of affect in order to gain insight into the manner in which cognition and affect specifically relate to one another to impact suicidal thoughts and behaviors. Other goals of the current study were to examine the relationship between affect and cognition in suicidal individuals, to determine if different patterns of affect exist for different subtypes of suicidal individuals (i.e., no suicidality, suicidal ideation only, suicidal behaviors), and to assess the unique role of affect in relation to cognition.

The National Suicide Hotline Designation Act, unanimously passed by both chambers of Congress and signed into law last year, designates the collection and use of 911-like fees to support 988 call centers, crisis outreach, and stabilization—the continuum of crisis care needed to appropriately respond to callers in distress. 988 service fees, like 911 service fees, will provide a stable, continued source of funding for the hundreds of under-resourced local crisis call centers that will answer 988 calls in communities across the country. An effective 988 crisis response system has never been more necessary. Before the outset of COVID-19, nearly 1 in 5 people lived with a mental health condition and over 10 million people experienced thoughts of suicide. Recent reporting from the Centers for Disease Control and Prevention indicate that mental health and suicidal ideation have worsened during the pandemic; approximately twice as many respondents reported serious thoughts of suicide and 40% of U.S. adults reported struggling with mental health or substance use. Present-day stressors can be particularly challenging for at-risk communities, with nearly 1 in 4 young people reporting suicidal thoughts.
