COURSE DESCRIPTION Normal and diseased periodontium including the structural, functional, and environmental factors. Emphasis on etiology, pathology, treatment modalities, and therapeutic and preventive periodontics. COURSE GOALS Upon completion of the curriculum, the student should be able to: 1. Relate the normal histological and anatomical structure of the periodontium to the pathophysiology of the periodontal disease process. 2. Identify risk factors, periodontal bacteria and level of virulence in periodontal disease. 3. Identify and describe the diseases of the gingiva and periodontium including the etiology and pathogens. 4. Recognize the pathological physiology of systemic disease and its relationship to oral health and dental hygiene treatment. 5. Describe the biological basis of occlusal function and its role in occlusal trauma and periodontal disease. 6. Define the goals and rationale for surgical and non-surgical periodontal therapy. 7. Describe the methods and relationships of assessment tools in periodontal disease. 8. Identify the sequence, effectiveness, and objectives of nonsurgical periodontal therapy. 9. Apply knowledge of the etiology and process of periodontal disease to the essentials of a treatment plan necessary to restore health to these tissues. 10. Develop a value system stressing the importance of prevention, the knowledge base and skills required to assess periodontal health and disease in the individual patient. 11. Demonstrate a synthesis and application of knowledge of periodontology in the recognition and treatment of diseases. CREDIT HOURS 3 credit hours CLASS MEETING TIME Monday and Wednesday – 8:00 – 9:15 INSTRUCTOR Michelle DeMoss, Instructor Office: 211 Multi Purpose Center Phone: 409-981-6814 6 Periodontology (DHYG 1311) Fall COURSE POLICIES: General Policy Statements: 1. Attendance Policy Absenteeism In order to ensure the students in the dental hygiene program achieve the necessary didactic and clinical competencies outlined in the curriculum, it is necessary that the student complete all assigned lecture classes, clinical and laboratory hours. If you are unable to attend lecture class, clinic or lab, it is mandatory that you call the appropriate instructor prior to the scheduled class, clinic or lab time. The student is responsible for all material missed at the time of absence. Extenuating circumstances will be taken into account. Extenuating circumstances might include: funeral of immediate family member, maternity, hospitalization, etc. It is expected that students will appear to take their exams at the regularly scheduled examination time. Make-up examinations will be given only if the absence is due to illness (confirmed by a physicians' excuse), a death in the immediate family, or at the discretion of the instructor. Fall/Spring Semesters: Dental hygiene students will be allowed two absences in any lecture, clinic or lab. Absences must be accompanied by a written excuse on the next class day. In the event that a student misses class, clinic or lab beyond the allowed absences, the following policy will be enforced: 2 absences = verbal warning 3 absences = written warning with the Disciplinary Action Form (DAF) 4 absences = grade will be lowered one full letter grade Summer or 8 week Sessions: Dental hygiene students will be allowed one absence in any lecture, clinic or lab. Absences must be accompanied by a written excuse on the next class day. In the event that a student misses class, clinic or lab beyond the allowed absences, the following policy will be enforced: 1 absence = verbal warning 2 absences = written warning with the Disciplinary Action Form (DAF) 3 absences = grade will be lowered one full letter grade 2. Tardiness Tardiness is disruptive to the instructor and the students in the classroom. It is expected that students will arrive on time for class, clinic or lab, and remain until dismissed by the instructor. If tardiness becomes an issue, the following policy will be enforced: Tardy 1 time = verbal warning Tardy 2 times is considered an absence. 3. Electronic Devices Electronic devices are a part of many individual’s lives today. Devices such as tape recorders, radios, telephones, and paging devices, however, may be disturbing to faculty and classmates. Students, therefore, must receive the instructor's permission to operate all electronic devices in the classroom and clinic. Texting on cell phones or computers will not be allowed during class. 4. Late coursework. Assignments must be completed by the due date. Late assignments will not be accepted and will result in a zero for that assignment. Americans with Disabilities Act (ADA). The Americans with Disabilities Act of 1992 and Section 504 of the Rehabilitation Act of 1973 are federal anti-discrimination statutes that provide comprehensive civil rights for persons with disabilities. O’Hehir, RDH, MS Hygienetown Editorial Director To some, the word “diagnosis” is taboo for hygien- ists to even consider using, let alone doing! Diagnosis is simply recognizing the signs and symptoms of disease, something all hygienists are required to do to take their licensing exam. Hygienists also must practice this in the clinical setting to provide care for patients. If a hygienist can’t tell the difference between health and disease, keeping a clinical position will be difficult. Those who don’t want RDHs to “diagnose” must instead want a robot to simply “scale teeth.” Every dentist I’ve know wants the RDH employed in the practice to “actually have a brain,” to quote Dr. Michael Rethman. Providing dental hygiene care involves critical thinking to assess the health of each individual patient. A wide variety of information is gathered to determine health, disease and individual risk factors presented by each patient. With the identi- fication of the dental hygiene diagnosis, the dental hygiene treatment plan can be devised and followed by the RDH. The dental hygiene diagnosis and treatment plan are part of the comprehensive dental diagnosis and treatment plan created by the dentist. Working as colleagues, the dentist and dental hygienist gather information necessary to accurately assess the health of each patient and provide the necessary treatment, prevention and maintenance care.

    Utah Code Chapter 69 Dentist and Dental Hygienist Practice Act Part 1 General Provisions 58-69-101 Title. This chapter is the "Dentist and Dental Hygienist Practice Act." Enacted by Chapter 116, 1996 General Session 58-69-102 Definitions. In addition to the definitions in Section 58-1-102, as used in this chapter: (1) "Asynchronous technology" means store-and-forward technology that allows a licensed dental professional to transmit a patient's health information to a dentist for viewing at a later time. (2) "Board" means the Dentist and Dental Hygienist Licensing Board created in Section 58-69-201. (3) "Dental assistant" means an unlicensed individual who engages in, directly or indirectly, supervised acts and duties as defined by division rule made in collaboration with the board.     Direct Access States The American Dental Hygienists’ Association (ADHA) defines direct access as the ability of a dental hygienist to initiate treatment based on their assessment of a patient’s needs without the specific authorization of a dentist, treat the patient without the presence of a dentist, and maintain a provider-patient relationship (ADHA Policy Manual, 13-15). Alaska 2008 Sec. 08.32.115 Collaborative Agreement: Dental hygienist may provide services according to the terms of a collaborative agreement. The dentist’s presence, diagnosis or treatment plan are not required unless specified by agreement. Care under the agreement can be provided in settings outside of the “usual place of practice” (i.e. private dental office). Requirements: Dental hygienist must have minimum of 4,000 hours of clinical experience within preceding 5 years. Agreement must be approved by state board of dental examiners. Dentists are limited to 5 or fewer collaborative agreements. Provider Services: Agreement can authorize nearly the entire dental hygiene scope of practice (patient education, prophylaxis, sealants, radiographs, etc). Arizona 2004/2015/ 2019 Sec. 32-1281, 32-1289 Affiliated Practice Agreement: Dental hygienist with a written affiliated practice agreement may perform dental hygiene services in specified settings outside the private dental office. The written agreement must be submitted to state board of dental examiners. The affiliated practice dental hygienist shall consult with the affiliated practice dentist before initiating further treatment on patients who have not been seen by a dentist within 12 months of the initial treatment by the dental hygienist. Requirements: Dental hygienist must have held an active license for at least 5 years and be actively engaged in dental hygiene practice for at least five hundred hours in each of the 2 years immediately preceding the affiliated practice relationship.

Once plaque hardens into tartar (calculus), the only way it can be removed is by a professional cleaning by your dental hygienist. We say that we are going to have our teeth cleaned — but a lot more than simple cleaning takes place during this visit! • - Your hygienist will ask you about your general health and your dental health and any recent changes in either. By doing so she will pinpoint any issues that re- quire special precautions during your cleaning. • - Next, the hygienist carefully examines the skin in and around your mouth looking for lumps, bumps, sores, tenderness or swellings and refers areas of con- cern to the dentist for further evaluation. • - Your hygienist will look closely at the state of your periodontal health. This includes checking your gums and the other tissues sur- rounding your teeth for inflammation (gingivitis) or bleeding. • - The hygienist will examine your teeth for decay. • - The hygienist will note the location and condition of stains or hard mineral deposits (calculus or tartar), then uses specialized tools to remove the deposits from your teeth. • - A mechanical polisher and polishing compound are used to polish the sur- face of your teeth so that they are smooth, making them more resistant to plaque, re- moving stains and leaving your teeth feeling squeaky clean. • - The hygienist uses a tiny probe to measure the space between your teeth and gums.

There are dental hygienists who provide periodontal procedures (SRP, PM) but document preventive procedures (AP). There are business staff who bill for adult prophylaxis when the hygienist has provided periodontal procedures. 2 Concerns? Both scenarios cause the practice to lose money. Both scenarios would be considered risk management issues.
