Welcome to Health Plan of Nevada! Welcome to Health Plan of Nevada! This directory includes a list of your plan’s network providers. At Health Plan of Nevada (HPN), we’re committed to providing you with access to quality health care coverage that is affordable and convenient. We know it’s important to build a trusted, long-term relationship with your primary care provider (PCP). It’s equally important to choose the right PCPs for you and your family. That’s why Health Plan of Nevada’s provider directory contains information to help you narrow your choices. Our provider directory offers the following key information: Names and addresses of contracted plan providers, including physicians, physician extenders, hospitals and pharmacies Area of specialty Board certification status Languages spoken (when available) Service limitations or offerings (i.e., adult patients only) Please know that physician extenders, such as physician assistants or nurse practitioners, are licensed in general practice. Although some may have additional training, the specialty they are listed under reflects the licensed specialty of their supervising physician. Remember, your PCP is responsible for ordering appropriate tests, submitting referrals to specialists, and requesting authorizations when needed. Please know that without the appropriate referrals and authorizations, you may be responsible for the cost of some services. You have the flexibility of changing to another provider at any time by calling Member Services at (702) 242-7300 or (800) 777- 1840, TTY 711, Monday through Friday from 8am to 5pm. Did you know every member of your family may choose a different PCP? You may select a pediatrician as your child’s PCP, and all female members ages 14 and older should choose an OB/GYN, in addition to their PCP. We truly value you as a member and look forward to being your partner in health. If you need assistance in selecting a provider, please call our Member Services Department. Information contained in this directory may change because of additions, deletions, or changes in practice locations and arrangements.
