4. Presentations

Jenny Fullard – NHS – Together Were Better

Together were better is one of 44 sustainability and Transformation partnerships (STP’S) in . Local NHS and Local Government organisations working with independent and Voluntary Organisations to transform local Health Care.

They are holding discussions with local communities based around the following key areas:

 Developing a new vision for health care across Staffordshire and Stoke on Trent  Simplifying urgent emergency care  Reviewing Community Hospitals in South Staffordshire  Identifying additional priorities that will deliver clinical and financial stability.

Forum Attendees all contributed to a discussion on topics such as GP Services in Rural Areas and travelling to appointments etc.

5. Kate Harrold – Stafford Borough Council

Stafford Borough Council Community and Wellbeing Partnership

There was a discussion on the Importance of Community and Voluntary Sector wellbeing.

There are 2 examples of priorities framework- Anti Social Behaviour and Homelessness

1. Signposting and Social Prescribing

2. Early intervention- routes for people on the cusp- request for mapping with Community Connectors

 A place based approach – think Highfields and There are to be 2 events focusing on Children and Families and will introduce older people. Aware of Data and how commissioning decisions made  Communities First Services are last approach  Ways of working to reduce demand into Stafford Services

Gemma Ward- – There is to be a Harm Reduction Hub, at Stafford Borough Council to discuss Anti-social behaviour.

Elaine Denney - Support Staffordshire - Updated on the Social Prescribing Project being delivered in Stone and Eccleshall. The project has been very successful with good engagement from the GP's, the target for the year was 80 with 120 reached by Quarter 3.

Hayley Smith- Housing Team – There is to be a review of the Homelessness, Rough sleeping which is out for consultation now until 20th June.
