    INFANT AND CHILD MORTALITY 8 This chapter presents estimates of childhood mortality, measured by perinatal, infant, and child mortality rates. The chapter includes a description of the indicators, the current levels and trends, differen- tials by selected background characteristics, and factors that contribute to elevating children’s mortality risks. Analyzing the levels and trends of childhood mortality is important in gauging the impact of ma- ternal and child health programs improving the health of infants and children. The data can also be used as input in population projections. Differentials in childhood mortality by selected characteristics are use- ful in identifying groups in need for priority attention and in planning meaningful strategies to address them. 8.1 DEFINITIONS AND ASSESSMENT OF DATA QUALITY Six indicators are analyzed in this chapter: Infant mortality rate, neonatal mortality rate, postneo- natal mortality rate, child mortality rate, under-five mortality rate, and perinatal mortality rate. The infant mortality rate (IMR) is defined as the number of infant deaths per 1,000 live births during the first 12 months of life. It is described as the probability of dying between birth and age one year. Because the level of mortality is higher at the early ages of infancy than at the later ages of infancy, it is useful to break up the rate into neonatal mortality (NN), the probability of dying within the first month of life, and postneonatal mortality (PNN), the probability of dying after the first month of life but before age one year. The child mortality rate is the probability of dying between exact age one and age five, defined as the number of deaths of children age 1-4 years per 1,000 children surviving to age 12 months.

    Gamlin and Holmes BMC Pregnancy and Childbirth (2018) 18:243 https://doi.org/10.1186/s12884-018-1870-6 RESEARCHARTICLE Open Access Preventable perinatal deaths in indigenous Wixárika communities: an ethnographic study of pregnancy, childbirth and structural violence Jennie Gamlin1* and Seth Holmes2 Abstract Background: Preventable maternal and infant mortality continues to be significantly higher in Latin American indigenous regions compared to non-indigenous, with inequalities of race, gender and poverty exacerbated by deficiencies in service provision. Standard programmes aimed at improving perinatal health have had a limited impact on mortality rates in these populations, and state and national statistical data and evaluations of services are of little relevance to the environments that most indigenous ethnicities inhabit. This study sought a novel perspective on causes and solutions by considering how structural, cultural and relational factors intersect to make indigenous women and babies more vulnerable to morbidity and mortality. Methods: We explored how structural inequalities and interpersonal relationships impact decision-making about care seeking during pregnancy and childbirth in Wixarika communities in Northwestern Mexico. Sixty-two women were interviewed while pregnant and followed-up after the birth of their child. Observational data was collected over 18 months, producing more than five hundred pages of field notes. Results: Of the 62 women interviewed, 33 gave birth at home without skilled attendance, including 5 who delivered completely alone. Five babies died during labour or shortly thereafter, we present here 3 of these events as case studies. We identified that the structure of service provision, in which providers have several contiguous days off, combined with a poor patient-provider dynamic and the sometimes non-consensual imposition of biomedical practices acted as deterrents to institutional delivery.

    1.0 Maternal and perinatal mortality 1.1 Definitions The Queensland Maternal and Perinatal Quality Council uses the following definitions. Fetal deaths (= stillbirth): Defined by the Registration of Births, Deaths and Marriages Act as a child whose heart has not beaten after it has been completely expelled or extracted from its mother and who is either of not less than 20 weeks gestation, or of not less than 400g by weight at birth. Livebirths: Defined by the Registration of Births, Deaths and Marriages Act as a child whose heart has beaten after it has been completely expelled or extracted from its mother. Mothers: Number of mothers is defined as the number of women having a pregnancy which resulted in a livebirth or fetal death. Maternal death: A maternal death is defined by the World Health Organisation (WHO) as the death of a woman while pregnant or within 42 days of termination of pregnancy, irrespective of the duration and the site of the pregnancy, from any cause related to or aggravated by the pregnancy or its management. This definition excludes deaths from accidental or incidental causes. The definitions used in this report include, in addition to the WHO definition, incidental deaths and deaths occurring more than 42 days after termination of the pregnancy, when their origin and illness related to the pregnancy2. Maternal mortality ratio: The maternal mortality ratio is defined as: Number of maternal deaths X 100,000 Number of mothers Late maternal death: Death of a woman within one year of giving birth or otherwise ending a pregnancy. These deaths are not included in the calculation of the maternal mortality ratio.

It was prepared by the Department of Health and Human Services, Centers for Disease Control and Prevention, National Center for Health Statistics (NCHS). These instructions pertain to the 2003 revisions of the U.S. Stan­ dard Certificate of Death and the U.S. Standard Report of Fetal Death and the 1992 revision of the Model State Vital Statistics Act and Regulations. This handbook is intended to serve as a model that can be adapted by any vital statistics registration area.
