FINCK, M.D. University Hospital, Boston, DeKalb General Hospital, Decatur, Georgia, and Boston University School of Medicine, Boston, Massachusetts 02118 INTRODUCTION SEVERAL reports have been presented that have discussed cardiovascular collapse following the intravenous infusion of succinylcholine in patients with burns, massive trauma,tetanus, spinal cord injury, brain injury,upper or lower motor neuron disease,and uraemia with increased serum potassium. The purpose of this article is to report a spinal cord injured patient who developed cardiac arrest following administration of succinylcholine, possibly due to succinylcholine-induced hyperkalaemia. The anaesthesia management during the course of subsequent surgical procedure proved to be uneventful. CASE REPORT On 7 December 1971, a 20-year-old white man was admitted to the hospital after he fell 50 feet from a scaffold to the ground. He was reported to have been in good health until this accident. His legs and outstretched hands absorbed the major impact. No loss of consciousness was reported at any time. Neurologic examination revealed absent function of muscle groups in the distribution distal to L4, including the sacral segments. There were contractions of both quadriceps muscles in the adductors of the legs. The legs were held in flexion with no evidence of function in his hip abductors, extensors, knee flexors or anything below his knee. He had intact sensation over the entire thigh and medial calf. He had no apparent abdominal or cremasteric reflexes, no knee or ankle jerks, no Babinski responses, and no sacral sparing. -

SPINAL INJURY Presented by:- Bhagawati Ray DEFINITION Spinal cord injury (SCI) is damage to the spinal cord that results in a loss of function such as mobility or feeling. TYPES OF SPINAL CORD INJURY Complete Spinal Cord Injuries Complete paraplegia is described as permanent loss of motor and nerve function at T1 level or below, resulting in loss of sensation and movement in the legs, bowel, bladder, and sexual region. Arms and hands retain normal function. INCOMPLETE SPINAL CORD INJURIES Anterior cord syndrome Anterior cord syndrome, due to damage to the front portion of the spinal cord or reduction in the blood supply from the anterior spinal artery, can be caused by fractures or dislocations of vertebrae or herniated disks. CENTRAL CORD SYNDROME Central cord syndrome, almost always resulting from damage to the cervical spinal cord, is characterized by weakness in the arms with relative sparing of the legs, and spared sensation in regions served by the sacral segments. POSTERIOR CORD SYNDROME Posterior cord syndrome, in which just the dorsal columns of the spinal cord are affected, is usually seen in cases of chronic myelopathy but can also occur with infarction of the posterior spinal artery. BROWN-SEQUARD SYNDROME Brown-Sequard syndrome occurs when the spinal cord is injured on one side much more than the other. It is rare for the spinal cord to be truly hemisected (severed on one side), but partial lesions due to penetrating wounds (such as gunshot or knife wounds) or fractured vertebrae or tumors are common. CAUDA EQUINASYNDROME Cauda equina syndrome (CES) is a condition that occurs when the bundle of nerves below the end of the spinal cord known as the cauda equina is damaged. -

Joaquim1,3 ABSTRACT Surgical treatment is well accepted for patients with traumatic cervical facet joint dislocations (CFD), but there is uncertainty over which approach is better: anterior, posterior or combined. We performed a systematic literature review to evaluate the indications for anterior and posterior approaches in the management of CFD. Anterior approaches can restore cervical lordosis, and cause less postoperative pain and less wound problems. Posterior approaches are useful for direct reduction of locked facet joints and provide stronger fixation from a biomechanical point of view. Combined approaches can be used in more complex cases. Although both anterior and posterior approaches can be used interchangeably, there are some patients who may benefit from one of them over the other, as discussed in this review. Surgeons who treat cervical spine trauma should be able to perform both procedures as well as combined approaches to adequately manage CFD and improve patients’ final outcomes. Sinan Bakir1,2*, Rolf Lefering3, Lyubomir Haralambiev1,2, Simon Kim1, Axel Ekkernkamp1,2, Denis Gümbel1,2 & Stefan Schulz‑Drost2,4,5 Preliminary studies show that clavicle fractures (CF) are known as an indicator in the severely injured for overall injury severity that are associated with relevant concomitant injuries in the thorax and upper extremity. In this regard, little data is available for the rarer injuries of the sternoclavicular and acromioclavicular joints (SCJ and ACJ, respectively). Our study will answer whether clavicular joint injuries (CJI), by analogy, have a similar relevance for the severely injured. We performed an analysis from the TraumaRegister DGU (TR‑DGU). The inclusion criterion was an Injury Severity Score (ISS) of at least 16. In the TR‑DGU, the CJI were registered as one entity. The CJI group was compared with the CF and control groups (those without any clavicular injuries). Concomitant injuries were distinguished using the Abbreviated Injury Scale according to their severity. The inclusion criteria were met by n = 114,595 patients. In the case of CJI, n = 1228 patients (1.1%) were found to be less severely injured than the controls in terms of overall injury severity. Compared to the CF group (n = 12,030; 10.5%) with higher ISS than the controls, CJI cannot be assumed as an indicator for a more severe trauma; however, CF can. Concomitant injuries were more common for severe thoracic and moderate upper extremity injuries than other body parts for CJI. -

The authors report a patient who was on oral anticoagulants because of mitral valve disease and who developed paraplegia from subarachnoid bleeding involv ing the cauda equina. The differential diagnosis, investigations and treatment of the cauda equina syndrome are described. Keywords: cauda equina syndrome; anticoagulants; subarachnoid haemorrhage; mitral valve disease. Case report A 32 year old woman from Chile presented with a complete paraplegia. She claimed that the paraplegia had developed progressively over 8 months. Initially she had paraesthesiae in her feet, followed by progressive paresis of both legs, beginning distally, over a period of 3 months. Two months after the onset of illness she complained of bladder incontinence. There was no history of trauma or low back pain. Clinical examination in our hospital revealed a flaccid paraplegia at L1 level, and loss of sensation from the groins to the feet, including saddle anaesthesia. The knee and ankle jerks were absent. The anal sphincter was atonic. She had an indwelling urethral catheter, and she was faecally incontinent. Myelography and a CT scan were carried out, and a space-occupying lesion at the level of T12-L4 (Figs 1, 2) was defined. Surgical ex ploration was done to determine the cause. -

Although it is usually considered as an emergency, there is still controversy in the literature regarding the optimal timeframe of surgical intervention in complete spinal cord and cauda equina injuries. CASE PRESENTATION: We report a case of a 24-year-old victim of a road traffic accident admitted with an L3 fracture causing complete cauda equina syndrome, who underwent early surgery within 12 h and made an extraordinary recovery (from AIS A to E). DISCUSSION: Although the timing of surgery in complete traumatic spinal cord injury and cauda equina syndrome remains controversial, this case highlights the importance of early surgical intervention even in complete injuries. Clinical signs evolved to progressive weight loss, anorexia, apathy, and isolation from the group. Physical examination showed tail hypotonia, perineal hypalgesia, rectal and bladder sagging compatible with signs related to cauda equina syndrome. Complementary laboratory and sonographic assessment, and necropsy confirmed the diagnosis of chronic renal failure (CRF), which was attributed to the ascending pyelonephritis. The examination of urine culture showed growth of bacteria of the genus Streptococcus sp. This is a rare case in the equine species where the lower motor neuron dysfunction led the development of infectious process in the urinary tract, progressing to renal chronic condition incompatible with life.
