    Prepared by ASERNIP-S NATIONAL INSTITUTE FOR CLINICAL EXCELLENCE INTERVENTIONAL PROCEDURES PROGRAMME Interventional procedure overview of Partial Left Ventriculectomy Introduction This overview has been prepared to assist members of IPAC advise on the safety and efficacy of an interventional procedure previously reviewed by SERNIP. It is based on a rapid survey of published literature, review of the procedure by specialist advisors and review of the content of the SERNIP file. It should not be regarded as a definitive assessment of the procedure. Procedure name Partial Left Ventriculectomy The Batista Procedure Specialty society Society of Cardiothoracic Surgeons of Great Britain and Ireland Executive Summary Left partial ventriculectomy (PLV) seeks to treat dilated cardiomyopathy by reducing cardiac volume and hence heart wall pressure through the resection of a portion of the left ventricle. Patients receiving it are generally suitable for cardiac transplant but unable to receive it, often for social or economic reasons. PLV is an emerging procedure and the vast majority of evidence is case series, although there has been one retrospective comparative study of PLV and transplantation. Hospital mortality was reported for up to 30% of patients and overall mortality was around 40% of patients. Thirty day survival ranged from 50% to 99%, and no significant difference was found between PLV and transplant. One-year survival ranged from 46% to 80% and three- year survival was reported in one study as 60%. Event-free survival was reported as 80% at 30 days, 49% at 1 year and 26% at three years. Use of a left ventricular assist device or relisting for cardiac surgery was reported in one study at between 5% and 15% of patients at 30 days and in 43% at 1 year and 58% at 3 years.

Abstract Heart disease is the prime cause of mortality around the world. The treatment for these is challenging. In this experiment, we have delivered fluorescent dyes via ASD into the epicardium and by epicardial injection and to document and compare the effects of two different modes of the distribution of fluorescent dyes in vital organs of the body. We found that In vivo imaging after the administration of the fat-soluble cy5 dye for 1h (5 rats) and 24 hours (5 rats), the rat's heart was extracted by injecting a large amount of physiological saline into the ventricle.

BACKGROUND Surgical LV volume reduction is under investigation as an alternative for, or bridge to, heart transplantation for patients with end-stage dilated cardiomyopathy. METHODS We measured P-V loops in eight patients with dilated cardiomyopathy before, during and two to five days after PLV. The conductance catheter technique was used to measure LV volume instantaneously. RESULTS The PLV reduced end-diastolic volume (EDV) acutely from 141 Ϯ 27 to 68 Ϯ 16 ml/m2 (p Ͻ 0.001) and to 65 Ϯ 6 ml/m2 (p Ͻ 0.001) at two to five days postoperation (post-op).

    Clinical Policy: Ventriculectomy and Cardiomyoplasty Reference Number: PA.CP.MP.56 Effective Date: 01/18 Revision Log Last Review Date: 03/19 Description Guidelines to determine medical necessity for ventriculectomy and cardiomyoplasty procedures as a treatment for severe chronic heart failure Policy/Criteria It is the policy of Pennsylvania Health and Wellness® that that ventriculectomy (Batista procedure) and cardiomyoplasty procedures are considered experimental and/or investigational and are therefore not medically necessary. Background Heart failure is the final common path of myocardial dysfunction in most types of cardiac disease. Treatment options for heart failure include both medical and surgical therapy and surgical treatment, including ventricular assist devices (VADs), coronary revascularization, valve repair or replacement, total artificial heart, and heart transplantation. Heart transplantation has become the standard treatment for eligible patients with severe, irreversible biventricular failure unresponsive to medical or surgical treatment. Several surgical approaches have been explored as alternative treatments for patients with end-stage heart failure. Surgical options to reduce the size of the enlarged left ventricle and improve cardiac function include partial left ventriculectomy, also known as the Batista procedure. Partial left ventriculectomy involves removing an elliptical section of the ventricle to improve cardiac output in patients who have severe chronic heart failure. Multiple studies have found minor improvements in measures of heart function and clinical status in the short term, with high mortality rates, high recurrences of symptomatic heart failure, and fatal arrhythmias (Stolf et al., 1998; Startling et al., 2000; Franco-Cereceda et al., 2001). As such, this procedure has fallen out of use (Fang, 2015).
