Statement of Purpose The name and address of the registered provider is: Dr Marshall & Partners Heaton Norris Health Centre Cheviot Close Heaton Norris Stockport SK4 1JX Telephone: 0161 480 3338 Website: www.drmarshallandpartners.co.uk Registered Manager: Dr C Marshall Practice Manager: Mrs M Wallis The practice is located in Heaton Norris, Stockport in single storey purpose built Community Health Centre which was established approximately in the early 1970’s. Patients can be seen in 7 consulting rooms, there is a shared reception and waiting area, a disabled toilet and a further patient toilet. Administrative accommodation and a staff room with kitchenette. Externally, the practice has a car park with 16 car parking spaces. There is a one identifiable Disabled parking space outside the Health Centre and patients can access the practice via a ramp at the front entrance. Under the Health and Social Care Act 2008 (The Care Quality Commission (Registration) Regulations 2009 Part 4), the registering body (Dr Marshall & Partners) is required to provide to the Care Quality Commission a statement of purpose. The Primary Healthcare Care Team The Partners Dr Christopher Marshall - MBChB, MRCGP, DGM Dr Kathryn Maguire - MBChB, MRCGP, DRCOG, DRSRH Dr Carolyn Gormley - MBChB, MRCGP, DRCOG, DRSRH Dr Amy Miller - MBChB, MRCG, DRCOG, DRSRH Salaried GP’s Dr Rizwan Ahmed - MBChB, MRCGP Dr Lauren Clarke - MBChB, MRCGP Practice Nurses Suzanne Turner - RGN Patricia Ashworth - RGN Health Care Assistant Kathryn Hamnett The practice covers Heaton Norris, Heaton Chapel, Heaton Moor, Heaton Mersey, North & South Reddish, Brinnington, Offerton, Cale Green, Edgeley, Shaw Heath, Greatmoor, Davenport The Regulated Activities under CQC are: The provision of medical checks and general medical services through daily routine and urgent appointments, home visits as required and all associated administration. Extended Hours Are worked Monday’s until 20.00 hours (except Bank Holidays) and the 2nd and 3rd Saturday of each month. The surgery is open five full days a week with no lunchtime or half day closing except the 4th Thursday for each month from 12.00 – 14.00 hours for staff training. Out of Hours is covered by our local provider Mastercall Out Of Hours Service. Management of Chronic Disease This encompasses a wide range of conditions which require long term treatment and care. Our priority is to ensure this care is on-going and appropriate; to this end we shall endeavour to review patients’ medication on an annual basis. Diabetic, Stroke, CHD and Respiratory appointments are available each day with the doctors or nurses. General Nursing Care Our nurses provide, smoking cessation advice, cervical screening, NHS checks, phlebotomy service, blood pressure monitoring and 24 hour blood pressure monitoring, spirometry, well person checks, ecgs, cervical screening, routine child immunisation and adult vaccination, travel advise and immunisations. Maternity Services Antenatal clinics are held by the community midwife with post natal care provided by the doctors. Child Health Surveillance Baby clinics are carried out by three the GP’s with support from the nursing team. Minor Surgery Is undertaken by one GP this includes minor surgical procedures and joint injections. General Information We endeavour to treat all our patients to the best of our ability and to do this with dignity, respect and honesty. All staff at the practice are committed to delivering a high quality service. We ask all patients to highlight any concerns and these are addressed professionally. Continuity of Care The practice aim is on building and maintaining a strong relationship between patients, all healthcare professionals and outside agencies as we feel this is essential to the way we work. This is especially so in the management of on-going problems or long-term illness we encourage patients to continue seeing the same doctor or nurse whenever possible. However, for a new problem if the doctor or nurse you normally see is not available then we would encourage you to see any of the doctors or nurses at the practice Information Governance We are fully compliant with the Data Protection Act and confidentiality is maintained at all times by the practice team. Learning and Development We are committed to providing training for all groups of staff this is done by way of an annual appraisal where learning and development needs are identified. Patient Participation Group The practice has a Patient Participation Group to engage with as many of our patients as possible. We aim to work with our patients to improve the service we deliver, including areas such as availability of appointments, clinical care, online facilities etc. We contact members of the group by email or letter and ask for their views and opinions.
