After referral by a health professional, a co-ordinator discusses the reasons for referral and develops a programme to match their aims and abilities. Referral to Time to Talk is through a GP, other health professional or self referral. The walks are designed to help you get fit and there are a variety of different levels of walks from Level 1 (beginner) to Level 5 (advanced) so there is something for everyone. The programme allows you to progress through the levels as your fitness improves. Al-Anon Family Groups hold regular meetings where members share their own experience of living with alcoholism. Al-Anon does not offer advice or counselling, but members give each other understanding, strength and hope. The service will offer advice and an individual exercise programme, where appropriate, to improve strength, balance and confidence to reduce the risk of further falls. Referrals are accepted from GPs and other health professionals. Princess Royal Hospital Balance and Bone Team (Haywards Heath) The team runs an outpatient service, specialising in balance problems and falls. The team provides diagnosis, treatment and management of care through medical clinics, exercise groups or individual treatments. Referrals are accepted from GPs and other health professionals. Queen Victoria Hospital Falls Prevention Service (East Grinstead) The team assesses people in their own home, looking at risk factors, and refer people to appropriate treatment based in their own home, the Rehabilitation Unit or into the eight-week Balance and Bone Group. A separate plan is being prepared for Copthorne Ward, which makes up the rest of Worth Parish. 2 The Neighbourhood Plan is a new type of planning document. It is the lowest tier of a hierarchy of development plans which include the Local Development Plan prepared by the District Council and the Government’s National Planning Policy Framework (NPPF). When approved, this plan will shape the evolution of Crawley Down village and the surrounding countryside. 3 The Plan has been prepared by a sub-committee of Worth Parish Council and is based on substantial consultation with the local population and local businesses to identify and develop solutions for key local issues. We hope you find this update helpful; we would like to run quarterly update sessions via. Zoom or Live stream to ensure we keep you informed of our progress to date and provide updates on key topics. Please read on to learn more about our progress to date. On behalf of the Mid Sussex Division of Modality Partnership, thank you for your support and feedback to date on the recent consultation relating to changes proposed to bring our four practices together to improve our services. Modality Partnership currently operates four practices in West Sussex: Judges Close Surgery, Ship Street Surgery, Crawley Down Health Centre and Park View Health Partnership. Over the past 12 months, our surgeries have been facing significant challenges and providing continuity of healthcare during the pandemic has made us work in very different ways. Clinical and administrative staff on all four sites have been working very closely together and this has reinforced our view that as a larger practice working within the Modality Partnership, we can deliver better, more flexible and effective care than if we were on our own. Our ambition remains that together we can offer more localised services to our residents, continue to provide safe and effective care and improve the working environment for our staff teams. We will do this through greater cooperation and mutual aid, which will ultimately improve the culture and personalised care we can offer to our patients. Feedback has been provided in several ways: online zoom meetings (with patients and also representatives from the Town Council), electronic and paper- based surveys.

  • Bolney to Lindfield – 8½ Miles
    Bolney to Lindfield – 8½ miles Cross the new A23 by means of the underpass, go straight ahead at the cross path and proceed downhill through woodland. This path is narrow and in places growth has obscured a rut or small ditch on the left so care is needed. The path emerges on to Buncton Lane. Turn left along the lane. Ignore the first footpath sign to the right at ‘Pickwell’ and take the next path on the right, TQ273233. Follow signs uphill to the north-east making for the distant house visible across two fields. A stile and a gate lead out to Bishopton Lane by the house, TQ277237. Cross at the stile opposite and enter Black Forest. Turn left (north) at the signposted 3 way footpath junction. Now proceed downhill into Long Wood and cross the stile into Broxmead Lane and turn right. After about 200 yards the lane turns sharp left, at this point take the footpath leading off to the right. Go down a short track and enter the vineyard through the gate. Cross the vineyard and go out of the next gate, turn right at the field boundary and follow the track to Deaks Lane, TQ288246. Turn left, past ‘Cuckfield Cattery’ and go right through a gate into a field opposite ‘The Wylies’. The route from here into Cuckfield follows the ‘High Weald Landscape Trail’. Continue down the slope to a bridge over a stream and ascend a track through woodland. Ignore a cross track. Follow signs along a chicken wire fence, then across the stile into the open with views opening up to the South Downs and the spire of Cuckfield Church ahead.
