The RoC contains a list of identified substances (i) that 3 either are known to be human carcinogens or are reasonably be anticipated to be human 4 carcinogens and (ii) to which a significant number of persons residing in the United 5 States are exposed. The Secretary, Department of Health and Human Services (HHS), has 6 delegated responsibility for preparation of the RoC to the National Toxicology Program 7 (NTP), which prepares the report with assistance from other Federal health and 8 regulatory agencies and nongovernmental institutions. 9 Nominations for (1) listing a new substance, (2) reclassifying the listing status for a 10 substance already listed, or (3) removing a substance already listed in the RoC are 11 reviewed in a multi-step, scientific review process with multiple opportunities for public 12 comment. The scientific peer-review groups evaluate and make independent 13 recommendations for each nomination according to specific RoC listing criteria. This 14 background document was prepared to assist in the review of aristolochic acids. The 15 scientific information used to prepare Sections 3 through 5 of this document must come 16 from publicly available, peer-reviewed sources. Information in Sections 1 and 2, 17 including chemical and physical properties, analytical methods, production, use, and 18 occurrence may come from published and/or unpublished sources. -

These plants have been used for decades to treat various diseases. However, the consumption of products derived from plants containing AA has been associated with the development of nephropathy and carcinoma, mainly the upper urothelial carcinoma (UUC). AA has been identified as the causative agent of these pathologies. Several studies on mechanisms of action of AA nephrotoxicity have been conducted, but the comprehensive mechanisms of AA-induced nephrotoxicity and carcinogenesis have not yet fully been elucidated, and therapeutic measures are therefore limited. This review aimed to summarize the molecular mechanisms underlying AA-induced nephrotoxicity with an emphasis on its enzymatic bioactivation, and to discuss some agents and their modes of action to reduce AA nephrotoxicity. By addressing these two aspects, including mechanisms of action of AA nephrotoxicity and protective approaches against the latter, and especially by covering the whole range of these protective agents, this review provides an overview on AA nephrotoxicity. It also reports new knowledge on mechanisms of AA-mediated nephrotoxicity recently published in the literature and provides suggestions for future studies. -

Herbal medicine involves the use of natural compounds, which have relatively complex active ingredients with varying degrees of side effects. Some of these herbal medicines are known to cause nephrotoxicity, which can be overlooked by physicians and patients due to the belief that herbal medications are innocuous. The genus has attracted so much great interest because of their numerous biological activity reports and unique constituents, aristolochic acids (AAs). In 2004, we reviewed the metabolites of Aristolochia species which have appeared in the literature, concerning the isolation, structural elucidation, biological activity and literature references. In addition, the nephrotoxicity of aristolochic acids, biosynthetic studies, ecological adaptation, and chemotaxonomy researches were also covered in the past review. In the present manuscript, we wish to review the various physiologically active compounds of different classes reported from Aristolochia species in the period between 2004 and 2011. In regard to the chemical and biological aspects of the constituents from the Aristolochia genus, this review would address the continuous development in the phytochemistry and the therapeutic application of the Aristolochia species. -

They are reported to be carcinogenic and nephrotoxic; and are implicated in kidney diseases, aristolochic acid nephropathy (AAN) which may result in kidney failure, other health complications and possibly death. Aristolochic acids are highly genotoxic and are linked to upper urothelial cancer in animals and humans. Some Aristolochia species are used in traditional medicine practice in Nigeria and other West African countries without regard to safety concerns. Several countries, especially in the Western world, have banned the use and importation of herbal products containing aristolochic acids. -

B. ARISTOLOCHIA SPECIES AND ARISTOLOCHIC ACIDS 1. Exposure Data 1.1 Origin, type and botanical data Aristolochia species refers to several members of the genus (family Aristolochiaceae) (WHO, 1997) that are often found in traditional Chinese medicines, e.g., Aristolochia debilis, A. contorta, A. manshuriensis and A. fangchi, whose medicinal parts have distinct Chinese names. Details on these traditional drugs can be found in the Pharmacopoeia of the People’s Republic of China (Commission of the Ministry of Public Health, 2000), except where noted. Herbal ingredients are traded using their common Chinese Pin Yin name and this can lead to confusion. For example, the name ‘Fang Ji’ can be used to describe the roots of Aristolochia fangchi, Stephania tetrandra or Cocculus species (EMEA, 2000). Misuse of Herbal Remedies: The Case of an Outbreak of Terminal Renal Failure in Belgium (Chinese Herbs Nephropathy) JEAN-LOUIS VANHERWEGHEM, M.D., Ph.D. ABSTRACT At least 100 cases of extensive interstitial fibrosis of the kidneys were observed in Belgium in women who had followed a weight-loss regimen that included the use of Chinese herbs. The possible relation between the renal disease and these Chinese herbs was investigated. It was shown that the prescribed Chinese herb called Stephania tetrandra was, in fact, inad vertently replaced by another Chinese herb, namely Aristolochia fangchi in the powdered ex tracts delivered in Belgium and in France. The development of renal disease in about 100 pa tients exposed to the so-called Stephania tetrandra stresses the need for more stringent control of herbal medicine. This paper considers how providers of high quality technical plant name information can better meet the requirements non-botanical audiences who also rely on plant names for elements of their work. The International Plant Name Index, World Checklist of Selected Plant Families and The Plant List are used as examples to illustrate the strengths and weaknesses of plant name resources from a non-expert user’s perspective. The above resources can be thought of as botanists pushing data at audiences. Without closer engagement with users, however, there is a limit to their relevance and impact. The need to cover common names is a frequent criticism of existing resources. The Medicinal Plant Names Services (MPNS, www.kew.org/mpns) is an example of how plant name resources can be adapted to better address the needs of a non-botanical audience. Some of the major challenges are outlined and solutions suggested. CHAPTER ONE INTRODUCTION 1 1.0. INTRODUCTION 1.1. BACKGROUND OF STUDY Plants continue to be a major source of medicines as they have been throughout human history. A 2008 report from the Botanic Gardens Conservation International (representing botanic gardens in 120 countries) revealed that 5 billion people still use medicinal plants to partly cater for their health care needs. According to the World Health Organization, medicinal plants would be the best source of a variety of drugs (Toroglu, 2011). The fact that plants synthesize a wide variety of chemical compounds that possess important biological functions accounts for this very important role of medicinal plants in health care. These chemical compounds known as phytochemicals, have been reported to possess beneficial effects on health on long-term basis and can be used to effectively treat diseases that affects humans. About 12,000 of such compounds that have been isolated so far have been estimated to be less than 10% of the total plant active ingredients available (Tapsell et al., 2006). Currently, plant-derived drugs constitute about 25% of conventional medications used today (Rao et al., 2004). Some of these drugs were obtained from plants reported to be potentially toxic. Examples of such drugs are colchicine from Colchicum autumnale used in the management of gout; and digoxin from Digitalis purpurea used in the management of heart failure. Some medicinal plants in their crude form have also been reported to produce better pharmacological activity than their isolated active components, and in some cases, their isolated active components are more toxic (CHEMEXCIL, 1992) or less efficacious (Kicklighter et al., 2003) than the crude extract. -

The increased number of patients with CKDu is becoming a burden on the health sector as treatments, dialysis, and organ transplant are costly procedures. There are many myths to describe CKDu; however, the real causative factor is remaining unknown. Some researchers believe that Neprotoxic herbal medicines containing Aristolochic Acid could be a factor for Nephrotoxicity incidences. Therefore, this study's primary objectives were to examine which species of Aristolochia plants exist in Sri Lanka, to list the species if any ingredients are of Aristalochia in traditional herbal remedies used in Sri Lanka and study the prescription pattern of medications containing Aristalochia. The data was collected by literature survey, field and ethnobotanical surveys, and focus group discussions. Results showed that few Ayurvedic practitioners use leaf, root, fruit, or plant parts of Aristolochia indica as a part of their remedies to treat more than twenty diseases and poison bites occasions. Also, nearly 66 prescriptions containing Aristolochia indica as an ingredient were found in the literature used by a few Ayurvedic practitioners in CKDu prevalence areas. Therefore, the research team concluded that nephrotoxic herbal medicines could be a reason for the current CKDu situations in Lanka.
