The introduction of specialist nurses in Malta in 2003, now titled ‘practice nurses’, was seen as an evolution in nursing practice and launched in response to gaps in services and developments in health policy. This was perceived as a much needed career advancement option for nurses; an alternative to traditional administrative/educational roles; and a drive towards improving patient access and quality care. However there was still a paucity of information and evaluation of the nurses’ role in Malta. The aim of this study was to explore the perceptions of the roles, development and preparation of specialist nurses in Malta, an island with its own historical, political and social context.

Given the ministry’s large size and complexity as well as the myriad of different professions and staff, it was felt appropriate that a new Human Resource Division be set up, incorporating three Directorates covering the full range of HR functions. The post of Director General Human resources however remains vacant after a first open call for applications. Over 2012, the Ministry has consolidated and expanded its commissioning function considerably. It now has several contracts in place with the private and NGO sectors. The main focus was for the provision of long term care beds, for investigative and therapeutic services linked to the waiting list initiative and for services provided by several NGOs. This is in line with the Ministry's policy to serve as funder and commissioner of services and to contract services in a cost-beneficial manner. This has also served to improve efficiencies within the Public Health Service and to decrease waiting lists significantly in some areas by contracting specific services to the private sector.

    Asylum Seekers and Primary Care in Malta: A Critical Exploration of Healthcare Encounters Submitted in partial fulfilment for the award of Doctor of Philosophy Joanna Depares De Montfort University December 2016 Abstract There is a substantial body of literature that highlights disparities in asylum seeker health and healthcare access and the difficulties that healthcare professionals face when providing care to asylum seekers and refugee populations. Few studies, however, have explored these processes within the context of Maltese society. This study aimed to gain an insight into cross-cultural healthcare practices through an exploration of clinical encounters between asylum seekers and healthcare professionals in the primary health care setting in Malta. The research design was guided by an ethnographic approach, with data collated through observation and in-depth interviews. The observational period consisted of one hundred and thirty hours spent mainly in a government Health Centre, with a focus on healthcare encounters between asylum seekers and healthcare professionals and included a series of informal conversations. Formal, semi-structured one-to-one interviews were carried out with 11 asylum seekers, two cultural mediators and seven healthcare professionals. Data were analysed inductively using thematic analysis informed by postcolonial and intersectionality theories within a candidacy framework. The analysis produced three main themes, namely: “Seeking Refuge and Beyond – The Migratory Process”; “Seeking Mutual Understanding – Communicating within the Healthcare Encounter” and “Seeking Resolution”. The first theme provides the asylum seekers’ poignant testimonials of their pre and post migratory experiences, including both the perceived impact of these on their physical and psychological health and the healthcare professionals’ perspectives of the asylum seekers’ migratory journey.

Accessibility and quality of health care services for everyone is a main issue in health policy. An adequate use of health care services is an important precondition for health. Little is known about health care utilisation of migrants, ethnic minorities and mobile populations in Malta. Research on the factors that affect health care utilisation of migrants is scarce and little is known about their needs, health behaviour and the possible barriers they experience in their use of health care services. Aims of the Study: The aim of this study was to document the key HIV, TB and HIV/TB co-infection mobility- related vulnerabilities of migrants in Malta through the conduction of a rapid mapping assessment of vulnerabilities and hotspots. It also aims to give an insight in the health care utilisation of migrants in Malta, the differences with Maltese nationals and the factors that can explain these differences.
