The purpose of this handbook is twofold. The first is to provide you with specific information about the Transition Center/Bridge staff, attendance and daily program expectations that are outlined on the first few pages of this handbook. Secondly, since we partner with Hinsdale South High School for all building level services and operations, this handbook, particularly pages 20-49, outlines detailed policies and procedures that all young adults must abide by as well. It is important that you become familiar with our handbook. Please read it and refer to it often throughout the year. By working together, we will promote opportunities for independence in the areas of: employment, post-secondary education and training, independent living skills and community access to better prepare your young adult/you for adult success. We strive to create a program that will meet the individual needs of the young adults/you and provide a safe and caring environment where all can learn. We are looking forward to a productive 17-18 school year in our new space. Septran Transportation Number: 630.357.1070 Visitor Sign- In/Sign- Out Procedure All visitors, including parents will be expected to sign-in/sign-out when arriving at the Transition Center, this includes visiting for IEP meetings and other check-ins. Please be prepared to show your State ID for identification purposes upon arrival. Authorized Pick-up/ Early pick-up In the event that someone other than the parent/guardian and/or individual(s) listed on the emergency contact form will be picking up your young adult, the Transition Center will require a signed written note, an email or phone call from the parent/guardian authorizing the young adult to be released to this individual. Please ensure that the provided communication includes: the name of your young adult the name of the individual who will pick-up your young adult, the date/time of the approved dismissal your signature, when applicable. **Please note that this designated individual, as well as your designated emergency contacts, will be required to show picture identification as well. Notification of School Closing Should it be necessary to close school, information will be posted on our website, and a message will be recorded on the school voice messaging system. In addition, school closure information will be available through local news and radio stations. ___________________________________________________________________________ District 86 Transition Center Daily Classroom/Community Expectations/Procedures ATTENDANCE EXPECTATIONS: Young adults are expected to arrive on-time for school unless they are excused. CONSEQUENCES FOR TARDINESS: 1. The first time a young adult is tardy for school, a verbal warning will be given to him/her. 2. The second time a young adult is tardy for school, a verbal warning will be given to him/her again. 3. The third time the young adult is tardy for school a parent will be notified. Repeated incidences (4 or more) will result in the loss of access/participation in Community Based Instruction. Alternative activities will be arranged within the Transition Center. If a continued pattern of tardiness behavior is established a meeting with staff, parents and young adult will be scheduled. ___________________________________________________________ CELL PHONE / ELECTRONIC DEVICE EXPECTATIONS: (Environments include the Transition Center, Community-based learning sites, Community-based outings) Use of cell phone/electronic device only during lunch/breaks unless directed by a staff member. Handle your own cell phone/electronic device. Do not touch other’s devices/phones. No pictures allowed unless consent is obtained. CONSEQUENCES FOR VIOLATION(s) Per incident: 1. Two verbal warnings will be given by a staff member to put the phone/device away. 2. The third time the young adult needs to be told to put the phone/device away, the young adult will be removed from the activity due to lack of compliance and a parent will be notified. The young adult’s phone/device will be taken away for the remainder of the day Repeated incidences/offenses (3 or more) will result in the loss of phone usage for 3 consecutive school days from 8am to 3pm (phone checked at the door upon arrival). If a continued pattern of unacceptable cell phone use is established a meeting with staff, parents and young adult will be scheduled to determine an individual plan regarding cell phone use. Any chronic or severe behavior may result in a Team meeting and contact to the Hinsdale South High School’s Dean’s office. The severity of a single incident may warrant an immediate or alternative consequence. ____________________________________________________________ COMMUNITY EXPECTATIONS: (Environments include the Community-based learning sites, Friday Outings, Restaurants, YMCA, Grocery Store, etc.) Follow directives from all staff members Participate as outlined in directives Use appropriate language Stay with assigned group Follow cell phone policy Be respectful to self and others CONSEQUENCES FOR VIOLATION(s) Per incident: 1. Two verbal warnings will be given by a staff member. 2. The third time the young adult needs to be told to follow community expectations they will be removed from the activity and if possible, brought back to the Transition Center. A parent will be notified. Repeated incidences (3 or more) will result in the temporary loss of access/participation in Community Based Instruction. Alternative activities will be arranged to be completed at the Transition Center. If a continued pattern of unacceptable behavior is established, a meeting with staff, parents and young adult will be scheduled. Any chronic or severe behavior may result in a Team meeting and contact to the Hinsdale South High School’s Dean’s office. The severity of a single incident may warrant an immediate or alternative consequence. ____________________________________________________________
