Adventure Camp 2017

Welcome Adventure Camp campers and parents! We have many exciting and adventurous activities planned for your child this summer. You’ll be pleased to know that we have a wonderful mix of new and returning staff. Please take a moment and get to know this year’s Adventure Camp staff.

My name is Luke Jones and I’m back for my 8th year as Director of Adventure Camp! When I’m not at Adventure Camp, I work for School District 103 in Lincolnshire as an Adaptive Physical Educator. I enjoy spending time with my wife, Liz and our 16 year old Aidan, 13 year old Lauren, and almost 1 year old Gavin!

Returning for her 2nd year is Logan Etheredge. She enjoys running and reading non-fiction novels. She will be attending University of Richmond in the fall. New to Adventure Camp this year is Christian Hurteau, Natasha Borkowski, and Rocco Cogliati. Christian just graduated from New Trier where he played and . He will be attending Colgate University in the fall. Natasha graduated from Loyola where she played and will be heading to Indiana University in the fall. Rocco is originally from Italy, where he coaches youth gymnastics, but is here for the summer!

ASSUMPTION AND ACKNOWLEDGMENT OF RISK WARNING: CLIMBING IS DANGEROUS!!! I, the undersigned, acknowledge and agree that the use of Climbing Apparatus and Instruction Activities sponsored by Vertical Endeavors have INHERENT RISKS. Those risks include, but are not limited to the following:

1. Injuries or death resulting from the failure or negligent misuse of Vertical Endeavors’ Climbing Apparatus.

2. Injuries resulting from slips, trips, falls, and/or the physical demands associated with the use of Vertical Endeavors’ Climbing Apparatus.

3. Injuries resulting from the swinging or fall of other persons who may come into contact with me or from any swinging or falls in which I come into contact with other persons.

4. Injuries occurring from the NEGLIGENCE or lack of adequate training of Vertical Endeavors’ volunteers or employees assisting with medical or other help either before or after injuries have occurred.

5. Injuries resulting from the failure of Vertical Endeavors Climbing Apparatus, and also including but not limited to, failure of ropes, slings, harnesses, belay devices, handholds, anchor points ,landing surface and its curbs, items left in landing surface and any other part of the climbing structure. 6. Injuries resulting from the NEGLIGENCE of Vertical Endeavors’ owners, operators, employees, or volunteer assistants,, the NEGLIGENCE of other climbers, visitors, or persons present at Vertical Endeavors, the NEGLIGENCE of the designers, manufacturers or installers of the Climbing Apparatus, and/ or the NEGLIGENCE Vertical Endeavors’ landlord. 7. If you engage in bouldering you further agree you will abide by and adhere to Vertical Endeavors’ rules and regulations with respect to bouldering and assume any and all risk associated with the failure to abide by those rules and regulations. If you desire to engage in Top Out Bouldering you will not do so until such time as you have to Vertical Endeavors’ satisfaction demonstrated the ability to engage in such activities and will not engage in Top Out Bouldering until such time as you have been provided the designated tag by Vertical Endeavors allowing you to engage in this activity. 8. Injuries resulting from Instruction Activities not directly related to climbing but related to other services offered by Vertical Endeavors in its facility.

I am aware of these and NUMEROUS OTHER INHERENT RISKS in using Climbing Apparatus and other activities offered by Vertical Endeavors including Instruction Activities. I FREELY AND VOLUNTARILY ASSUME COMPLETE RESPONSIBILITY for these risks and for the injuries that may occur as a result of these risks EVEN IF injuries occur in a manner not foreseeable at the time I sign this agreement. I realize that by voluntarily assuming the risks involved, I am SOLELY RESPONSIBLE for any loss or damage I sustain, including PERSONAL INJURIES to me, damage to my PROPERTY, or damage arising out of my DEATH.

RELEASE, PROMISE NOT TO SUE AND REPRESENTATIONS In consideration of my observing or using Vertical Endeavors’ Climbing Apparatus, and/or in consideration of my participating in Instructional Activities I, on behalf of myself, my heirs, administrators and personal representatives, hereby RELEASE VERTICAL ENDEAVORS AND FOREVER DISCHARGE IT FROM ANY AND ALL LIABILITY, and PROMISE NOT TO SUE Vertical Endeavors, or any of its officers, directors, employees, volunteers, or agents or any other climber, visitor, or person present in or using Vertical Endeavors’ Climbing Apparatus for any claims, losses, damages and/or demands arising out of any PERSONAL INJURIES sustained by me, damage to my PROPERTY, or my DEATH. This RELEASE extends to and shall be applicable to the designers, manufacturers and/or installers of Vertical Endeavors’ Climbing Apparatus and Vertical Endeavors’ landlord.

You represent you are over the age of 18 and that you have the demonstrated ability to top out on the bouldering apparatus at VE. You further agree not to engage in flips, tricks, jumping, or any other activity that is not safe in connection with bouldering.

All parents bringing children to the facility hereby acknowledge and agree they have been advised by VE staff of VE’s child supervision rules, I have reviewed the child supervision rules posted by VE within the facility and agree to abide by and follow the child supervision rules.

If any provision of this Agreement is held invalid, the invalidity shall not affect other provisions of the Agreement which can be given effect without the invalid provision, and to this end the provisions of the Agreement are severable. This Agreement shall be governed by the laws of the State of Minnesota.

ACKNOWLEDGEMENT AND RELEASE

For and in consideration of the admission price and the permissionto use the WHIRLYBALL Facilities you hereby agree to and acknowledge the following:

That you are only authorized to use the WHIRLYBALL premises and facilities upon the proper execution of this document. That this document is a material part of the consideration and agreement between you and WhirlyBall for the admission to and use of its premises and facilities. That you have read this entire document and completely understand its contents. You also acknowledge that any questions about the content of this document or use of the premises and facilities have been answered by a representative of WHIRLYBALL. That you accept all responsibility for property damage to WHIRLYBALL’S premises and facilities caused by your actions. You also understand that the term “WHIRLYBALL’S premises and facilities” includes the premises, all fixtures in and around it, the surrounding parking area and the equipment associated with it. That you acknowledge that the owners and operators of WHIRLYBALL have employed diligent efforts and care in making the facilities and premises as safe as possible; and that despite the foregoing efforts, you understand that there are, nevertheless, certain inherent risks in using the facilities and premises. Among these risks are possible trips, falls, collisions with other people or objects, etc. You specifically acknowledge the existence of these risks and agree to the use of the premises and facilities with the full understanding of the same. RULES OF THE ROAD

There are two ways to acquire points. One is by making the shot, which is good for two points, three points from half court. The second is by acquiring penalty points assessed against the opposing team. (In all cases, penalty points are awarded to the team not guilty of committing the penalty. Play does not stop.) The referee officiates from a mezzanine above the court and will keep score and call all penalties.

Four Point Penalty for collisions with excessive speed. The penalty goes against the person most responsible for the collision. Referee judgment call.

Four Point Penalty for pushing off with feet or hands against an opponent's WhirlyBug™ or wall.

Two Point Penalty for touching the ball with your hands unless it is wedged inside your scoop and is stuck there; or unless it goes inside your WhirlyBug™.

Two Point Penalty for goal tending. Raising up in your seat to block a shot so as to be off yourseat. You must keep your seat belt snug.

Two Point Penalty for trying to free up the ball when it has been covered by opponent's scoop on the floor or against the side of his/her WhirlyBug™.

Head on collisions and directed collisions from behind are determined by a judgment call from the referee as to who is more responsible for its occurrence.

In the event the ball gets smashed, the referee will make a determination as to which team last touched the ball, should a new ball be needed.

WhirlyBall™ Safety

No Running Permitted on the Court or in the Common Area.

No Admittance to the Court Without WhirlyBall Staff Supervision.

Keep One Hand on the Steering Column at All Times.

Seat Belt Must Remained Fastened at All Times.

No Play Allowed on the court Unless From Operating WhirlyBug™.

Do Not Use Hands to Pick Up the Ball From the Court Floor.

Do Not Use Hands or Feet to Push off Wall, Bumper, or Other WhirlyBug™.

Ramming from behind will not be tolerated and Player will be Ejected from the court.

Do not drive full speed into the wall or another WhirlyBug™, remove foot from accelerator pedal and turn to minimize impact.

Do not bump another player while they are scooping up the ball from the court.

No eating, drinking or smoking on the court.

The game of WhirlyBall™ is inherently dangerous and injury is possible even if all rules are followed. If you are not willing to assume the risk of injury please do not participate.

Failure to follow all rules or instructions may result in injury to yourself and others.

PARTICIPANT RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT ***READ BEFORE SIGNING*** In consideration of being allowed to participate in any way in the sea kayaking in the Chicago metropolitan area, related events and activities, I, ______, the undersigned, acknowledge, appreciate, and willingly agree to comply with terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately. I understand that in joining this Northwest Passage Outing Club, Inc., (hereafter “NWP”) Outing, I will be faced with certain inherent risks and exposures to danger in geographic areas which may be remote from facilities where medical treatment can be obtained. I the undersigned, acknowledge, appreciate and agree that the risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death. Such hazards may include, but are not limited to, falling, falling objects, drowning, waterway obstructions, elevation changes, waterfalls, rapids, fast currents, rocks, temperature exposure (hypothermia, sunstroke, sunburn, heat exhaustion), equipment failure, improper use of equipment, inadequate or improper instruction or assistance, failure to wear protective clothing including life jackets, weather conditions including electrical storms, poor water quality, exposure to wild animals, insects, collapse of natural or man-made structures, potable water contamination, upset, inadequate or improper swimming skills (including lifesaving), and experience of participants and trip leaders. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the release’s or others, and assume full responsibility for my participation. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless NWP, its officers, officials, agents and/or employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event, (collectively “Releasees”), from any and all claims, demands, losses, and liability arising out of or related to any injury, disability or death I may suffer, or loss or damage to person or property, whether arising from the negligence of the release’s or otherwise, to the fullest extent permitted by law. Should I be injured on this Outing I understand that various expenses may be incurred in rendering rescue operations and medical care, and I agree that I will be responsible for any such costs of evacuation procedures, rescue operations and emergency or non-emergency medical services which may be occasioned by my participation in the Outing. In making application to participate in the Outing, I affirm that I am in good health, capable of performing the required exercise to participate, and that I accept, at my personal risk, the hazards of such participation in the Outing and will not hold NWP, its members, other participants or its representatives responsible. In consideration of NWP accepting my application, I hereby release and forever discharge NWP, its officers, directors, servants, employees and agents and the other participants from any liability whatsoever arising as a result of my participation in the Outing. This release is binding upon me, my heirs, executors, administrators, and assigns.
