The reports and assessments provide organizations with comprehensive, science-based information on common, costly medical conditions and new health care technologies. This project aimed to collect and critically review the existing evidence on practices relevant to improving patient safety. Search Strategy and Selection Criteria: Patient safety practices were defined as those that reduce the risk of adverse events related to exposure to medical care across a range of diagnoses or conditions. Potential patient safety practices were identified based on preliminary surveys of the literature and expert consultation. This process resulted in the identification of 79 practices for review. The practices focused primarily on hospitalized patients, but some involved nursing home or ambulatory patients. Protocols specified the inclusion criteria for studies and the structure for evaluation of the evidence regarding each practice. Pertinent studies were identified using various bibliographic databases (e.g., MEDLINE, PsycINFO, ABI/INFORM, INSPEC), targeted searches of the Internet, and communication with relevant experts. Data Collection and Analysis: Included literature consisted of controlled observational studies, clinical trials and systematic reviews found in the peer-reviewed medical literature, relevant non-health care literature and “gray literature.” For most practices, the project team required that the primary outcome consist of a clinical endpoint (i.e., some measure of morbidity or mortality) or a surrogate outcome with a clear connection to patient morbidity or mortality. This criterion was relaxed for some practices drawn from the non-health care literature. The evidence supporting each practice was summarized using a prospectively determined format. The project team then used a predefined consensus technique to rank the practices according to the strength of evidence presented in practice summaries. A separate ranking was developed for research priorities. Main Results: Practices with the strongest supporting evidence are generally clinical interventions that decrease the risks associated with hospitalization, critical care, or surgery. Many patient safety practices drawn primarily from nonmedical fields (e.g., use of simulators, bar coding, computerized physician order entry, crew resource management) deserve additional research to elucidate their value in the health care environment. The following 11 practices were rated most highly in terms of strength of the evidence supporting more widespread implementation. • Appropriate use of prophylaxis to prevent venous thromboembolism in patients at risk; • Use of perioperative beta-blockers in appropriate patients to prevent perioperative morbidity and mortality; • Use of maximum sterile barriers while placing central intravenous catheters to prevent infections; v • Appropriate use of antibiotic prophylaxis in surgical patients to prevent postoperative infections; • Asking that patients recall and restate what they have been told during the informed consent process; • Continuous aspiration of subglottic secretions (CASS) to prevent ventilator-associated pneumonia; • Use of pressure relieving bedding materials to prevent pressure ulcers; • Use of real-time ultrasound guidance during central line insertion to prevent complications; • Patient self-management for warfarin (Coumadin™) to achieve appropriate outpatient anticoagulation and prevent complications; • Appropriate provision of nutrition, with a particular emphasis on early enteral nutrition in critically ill and surgical patients; and • Use of antibiotic-impregnated central venous catheters to prevent catheter-related infections. Conclusions: An evidence-based approach can help identify practices that are likely to improve patient safety. Such practices target a diverse array of safety problems. Further research is needed to fill the substantial gaps in the evidentiary base, particularly with regard to the generalizability of patient safety practices heretofore tested only in limited settings and to promising practices drawn from industries outside of health care. Gossypiboma is composed of non-absorbable surgical material with a cotton matrix. Gossypiboma is usually under-reported and is a severe medicolegal issue. Thus, we describe the computed tomography (CT) findings of gossypiboma in our institution. From January 2003 to June 2006, gossypibomas diagnosed in our institution and their data regarding sex, age, previous operation, location, the interval between the operation and the diagnosis of gossypiboma, clinical presen- tation, indication of CT, CT findings and further management were collected. There were 6 cases of gossypiboma, 4 men and 2 women. Three of our cases had previous chest surgery and the other 3 cases had previous abdominal surgery. The locations of 3 (50%) cases were in the left anterior subphrenic space. The mean interval between original operation and diagnosis was 24.6 ± 33.4 months (range, 17 days to 8 years). With regard to CT findings, 3 (50%) cases had an isodense mass and 3 (50%) had a typical mass containing curvilinear opaque structures. The mean size of the gossypibomas was 62 × 62 × 67 mm. Because gossypiboma is due solely to human factors and is a severe medicolegal issue, continuous education should be considered. It is an infrequent but avoidable surgical error. The retained sponge induces two types of reactions, fibrinous response resulting in granuloma formation and exudative response leading to abscess formation. This serious medical condition may result in significant morbidity and mortality with serious medico legal implications. We present two cases of retained guaze(gossypiboma) seen in a busy surgical unit within three months. The pathogenesis is due to gauze induced adhesions that may cause intestinal obstruction and abscess formation resulting in peritonitis . The plain abdominal radiograph was very valuable in the first line investigation of these patients. It is possible that gossypiboma is underreported and standard protocols are not common except for routine concern for detail while doing laparotomy. We present our experience with this entity. We report a case of 21 year old female who came to the OPD with off and on abdominal pain since 3 years with LMP 1 month back with regular normal flow, unmarried, sexually not active, with history of open appendicectomy 3 yrs back. USG report shows cholelithiasis with right sided complex ovarian cyst approximately 7cm and left ovarian simple cyst measuring 5.5 cm and CT SCAN shows Complex cystic mass in both adnexal region measuring 7cm and 5.5cm respectively, possibility of endometrial cyst. She was taken up for laparoscopic ovarian cystectomy and cholecystectomy. Ovarian mass decompression with extraction of gauze piece from right cyst with puncturing of the haemorrhagic left cyst and cholecystectomy was performed.

    The High Cost of Inaction How retained surgical sponges harm hospital finances and reputations 1 Executive summary It’s not something that any healthcare leader or medical professional wants to see on local TV or splashed across a newspaper’s front page: A 38-year-old patient at a California hospital files a lawsuit following the grisly discovery of a large lap towel left in her abdomen two years after having had surgery at the hospital.1 The standard sponge count done after the surgery had been falsely reported as correct. A second surgery has had to be conducted to remove the mass, which involved severe bowel adhesions. Part of her uterus, including her fallopian tubes, was incorporated into the mass. Surgeons were able to free her small bowel and sigmoid colon from the lap pad, and she was treated with intravenous antibiotics for 48 hours. The patient lost a section of her colon. The woman’s insurance company, citing a policy in its contract with the hospital, refused to pay for the second surgery to remove the mass. The state swept in to investigate the incident and fined the hospital $100,000, providing the full documentation of its probe to the media. The case had to be reported as a sentinel event to the Joint Commission and a root cause analysis and report on corrective actions compiled. A financial settlement was reached with the patient. In the unique nomenclature of healthcare, surgical sponges mistakenly left inside patients following surgeries are called “retained surgical items” or “retained foreign objects.” Sponges make up about two-thirds of all objects left inside patients; the remainder are wires, pieces of surgical tools and sometimes full instruments.2 As the case above shows, for such benignly named events, retained sponges often have devastating results.

This review highlights the risk factors and index pathology, as well as markers that raise clinical suspicion, of a condition that may be elusive in presentation on account of its otherwise nonspecific signs and symptoms. A review of the English literature reporting retained abdominal swabs between 1992 and 2012 revealed 100 cases. Fifty-six percent of patients presented with pain, most commonly coupled with an abdominal mass or symptoms of bowel obstruction; 6% of patients presented with a fistula or a sinus; and 6% presented with extrusion of the swab; only 7% presented with signs indicative of sepsis. The most common initial surgery was obstetric and gynaecological (in 44% of cases); the second most common was general surgery (36%), most commonly following cholecystectomy. Plain abdominal X-ray was done in 45% of patients, followed by ultrasound, computed tomography (CT) scan or both. CT scan is the best preoperative diagnostic tool currently. The varying presentations exhibited by this postsurgical entity will continue to perplex the attendant practitioner. Clinical suspicion assisted by ultrasound and CT scan will improve definitive diagnosis. While there are many checkpoints to prevent this rare yet significant complication, human error and the unpredictability of surgery may make elimination impossible. The challenges presented with a retained swab, although rare, will persist, and with it the devastating consequences for both patient and clinician.

Roberta Shanman, M.S. 4 Prevention of Wrong Site Surgery, Retained Surgical Items, and Surgical Fires Evidence-based Synthesis Program PREFACE Quality Enhancement Research Initiative’s (QUERI) Evidence-based Synthesis Program (ESP) was established to provide timely and accurate syntheses of targeted healthcare topics of particular importance to Veterans Affairs (VA) managers and policymakers, as they work to improve the health and healthcare of Veterans. The ESP disseminates these reports throughout VA. QUERI provides funding for four ESP Centers and each Center has an active VA affiliation. The ESP Centers generate evidence syntheses on important clinical practice topics, and these reports help: • develop clinical policies informed by evidence, • guide the implementation of effective services to improve patient outcomes and to support VA clinical practice guidelines and performance measures, and • set the direction for future research to address gaps in clinical knowledge.

Key words: Gossypiboma – Foreign body – Renal cell carcinoma – Bladder cancer Abstract: Gossypiboma refers to a retained foreign object that was forgotten in the body cavity during an operation. It is a rare surgical complication that most commonly occurs after intraperitoneal abdominal emergency surgical procedures, but may also occur after virtually any type of operation. Gossypiboma can be confused with neoplastic lesions and abscess. Clinical examination and radiological findings may sometimes mislead the physician. We intend to present our cases, which is thought to be a kidney tumour and bladder cancer but resulted gossypiboma which is a condition that is caused by a forgotten sponge during the operation and it can mimic the cancer. During the operation, the team must work in coordination and be careful. Unnecessary operations in such situation can significantly increase the patient’s morbidity.     A 65-Year-Old Man With an Endobronchial Gossypiboma After Lobectomy for Abscessing Pneumonia Michael Kreuter MD, Ralf Eberhardt MD, Matthias Wiebel MD, Martin R Schulz MD, Klaus-Michael Mueller MD, and Felix JF Herth MD We present a case of a 65-year-old man with recurrent hemoptysis and weight loss for 6 months. Thirty-two years earlier, lobectomy of the right lower lobe had been performed for abcessing pneumonia. Due to recurrent pulmonary infections after lobectomy the patient had to retire at the age of 46. A diagnostic procedure to explain the hemoptysis was performed. A computed tomogram revealed a suspicious formation in the bronchus intermedius, and the patient was referred to our department with the suspicion of lung cancer. During bronchoscopy an endobronchial mass was detected and extracted whole with a foreign-body forceps. Textile fibers of a sponge in the histology specimen led to the final diagnosis of gossypiboma (also known as textiloma). After removal of the gossypiboma no further pulmonary infections occurred.
