Discussion:

 Consumer Satisfaction Survey (handout): (email Marc suggestions to be finalized @ next Leadership Meeting) All services at Nisonger should seek feedback on quality of service from their patients/consumers. o BSS (appts. not relevant) o A lot of clients are accompanied by a driver only, who would not be able to complete the survey. o Form could be adapted by project/program (instead of ‘Nisonger Center’) o Survey Monkey (could be adapted for each program) instead of mailing? o Sara, Karen, Betsey, Susan are already using some type of CSS. o Use this as a model if you’re not already using one. o Should we say: On your most recent visit (this day… this Dr….)? o Change date sent to date seen o Should we include/not include the donation box at the bottom? . Use a separate card instead of placing it on the form (same size as the thank you notes).

 Dodd Hall Loop:

o Email Marc - # of spots requested; how many patients are seen daily (by the end of this week). Meeting was adjourned at 10:00 a.m. Minutes respectfully submitted by Tamara J. Hager
