Referencing PBM-MAP-VPE Posted Clinical Guidance Documents,
Educational Materials, and VA/DoD Clinical Practice Guidelines
1. Clinical Guidance documents developed by the Pharmacy Benefits Management Services, Medical Advisory Panel and VISN Pharmacist Executives should be referenced using the following format:
Title of guidance. Washington, DC: Pharmacy Benefits Management Services, Medical Advisory Panel and VISN Pharmacist Executives, Veterans Health Administration, Department of Veterans Affairs; Month Year.
Example:
Dalfampridine (Ampyra®) Criteria for Use. Washington, DC: Pharmacy Benefits Management Services, Medical Advisory Panel, VISN Pharmacist Executives and MSCoE Directors, Veterans Health Administration, Department of Veterans Affairs; September 2010.
2. National VA/DoD joint guidelines developed through the Office of Quality and Performance should be referenced using the following format:
Title of guideline. Washington, DC: Office of Quality and Performance and the Veterans Affairs and Department of Defense Development Work Group, Veterans Health Administration, Department of Veterans Affairs; Month Year. Office of Quality and Performance publication XX-XXXX.
Example:
Management of Substance Use Disorder in the Primary Care Setting. 3. National VA/DoD joint Practice guidelines developed through the Office of Quality and Performance (electronic version) should be referenced using the following format:
Name of Working Group. (Year). Title of guideline. Electronic Version. Washington, DC: Veterans health Administration and Department of Defense.
Example:
