As a parent/guardian I acknowledge that I have read the Southridge Youth Competition Team Information Packet for the 2014-2015 Season. I understand and endorse the expectations and requirements set forth in the packet for my daughter to be a member of the SYC Competition Team. I have read the financial expectations for being a part of the SYC Comp Team. I understand that if my daughter is selected for the team, I will be responsible for paying these expenses in order for my daughter to be allowed to participate.
