By signing below I agree that I have borrowed the dishware items listed and will return kits, clean and complete, by the time and date specified in this agreement. I agree to replace any item that is missing or damaged.

I understand that while Lane County Waste Management requires that all dishware be returned in clean condition, Lane County makes no representations or guarantees as to the cleanliness of the dishware. Lane County Waste Management recommends that all dishware be washed and sanitized before use. Lane County is not liable for any damages or injuries that may arise from the use of the dishware, and I agree to indemnify, defend and hold harmless Lane County, its officials, agents and employees from and against any and all claims, damages, losses and expenses, including legal fees arising out of or in any way associated with the use of the dishware made available by Lane County Waste Management. By my signature below, I acknowledge I have received a copy of this agreement.

This kit was developed by a Master Recycler volunteer. It is loaned for use at events on the honor system. Borrowers are expected to return items in a sanitary condition and replace any lost items. If there are doubts about the cleanliness, we recommend that you wash items before use.

INSTRUCTIONS: 1. Count the bin contents before use and note any difference with the previous count. 2. After use, replace items in clean condition---use mechanical dishwasher -OR- hand wash with sanitizing rinse (one capful of bleach per gallon of water) and let air dry. Launder napkins. 3. Count and record quantity of each item in new column before returning this kit.
