True Cost and Value of Mental Health Integration: Intermountain Healthcares’ Team-Based Approach to Population Health
Mini Course Outline
Course Objectives:
1) Identify how integrating mental health services into personalized primary care (medical home) improves outcomes for patients and families managing multiple and complex conditions and determines the total cost of effective team care
2) Demonstrate the greater value provided by high-performing, team-based care compared to the more traditional patient management approach, as measured by outcomes, costs, and utilization
3) Apply team dosing methodology to longitudinal chronic disease registries to monitor clinical and operational benchmarks for population health
Pre-work [choose one or both]
1) a- Interview colleague, administrator or staff person ( 30 min), b- Record their response to the following questions, c- Group the responses into ‘value’ themes, d – assign an outcome that would measure the impact of this value being achieved
