It is YOUR responsibility to ensure that a MAINTENANCE OF WELDER CERTIFICATION form is completed and received by the Apprenticeship Office on or before the * REQUIRED DATE. The Joint Apprenticeship and Training Trust will no longer mail a reminder to you indicating that your certification requires maintenance. If this maintenance is not completed by the *REQUIRED DATE, your welding certification(s) will expire. Directions for renewing your welding certification:

1. Complete the MAINTENANCE OF WELDER CERTIFICATION form  Download form at www.smw20training.com 2. *REQUIRED DATE:  GMAW – June 30th  GTAW – June 30th  ASME, Brazing – June 30th  FCAW – June 30th and December 30th  SMAW – June 30th and December 30th
