Minutes of the Meeting Monday 20th July 2015

Thanks for the Getting well card said Stephen

Jenny has had a reply from her MP who has had a reply from Ministry of Employment (there is no category for care on the employment website). As there is no money, nothing is being done. Jenny will persist. There is no point having My Life My Way if you cannot get the staff.

Sally and Jenny have gone to workshop on Pas.

Jenny is still advertising through Independent Lives but there is no quick fix.

Sally said that the sports club celebrated it 10th year anniversary and Peter Hull was presented with a plaque. He is leaving HCC in August.

1 Shared Lives – Lynda French

Leaflets are being printed and will be sent to Flo for the next LIG meeting in September.

Shared Lives supports LD, mental health, physical disability and older people who have dementia with respite day care.

Each area has a shared lives manager.

There is a big review managed by Claire Hudson. She is checking every process and is working with the commissioning team. A case review has shown that they need to increase the team. The review will comprise of a marketing plan. Claire will replace Mandy Bullock when she leaves in July.

It can take up to 6 months to recruit a carer but would like to cut the time by 3 months maximum. It will be a robust system too.

There are 250 shared lives carers in Hampshire (not Southampton).

They are working closely with the transition team.

Soo said that people really like Shared Lives but there are not enough families to chose from.

Sally liked Shared Lives but there is no choice and no flexibility so you need to plan one year ahead.

Some figures for Fareham and Gosport current Shared lives carers  17 carer households supporting around 40 service users.  There are 29 long stay placements with 1 active vacancy.  2 respite placements

Lynda to come back on 19th October (confirmed). Claire cannot come.

2 Partnership Board matters

We worked on My Day, the election of the Partnership board co-chair.

(Marcia was elected; she has a lot of experience).

Some good news: there will be money to support the re-launch of Safer Place scheme

Some issues with the new framework. Issues should be passed to care management team. Please don’t wait til the Board to pass on issues.

The LD reps report is now on line and was sent to the team managers in each area. Stuart Outterside contacted Amanda Kent when he saw some of the comments made by some service users. Some issues are now being sorted.

Safeguarding issues: more reports so people are likely to be more vigilant.

Health

Hello from Marta who covered the North Hampshire Team Basingstoke and Winchester hospitals.

She said that the team is planning 2 new health events in Basingstoke and Winchester.

She also said that there is LD awareness on training days in Basingstoke and Winchester even though there are no liaison hospital nurses currently.

3 Parents/carers commented that all hospitals should have the same passport – one consistent passport across Hampshire even across the Country would be good

Questions to Michelle Stickland Are hospitals providing support for LD staff to our LD people? Are all GP surgeries now signed up to provide annual health checks? Process for planned admissions for LD – is all staff aware? How many people have a WRAP? How many people have a care plan? What are the facilities for the over 18 in the hospitals (for one night and over?)

Bernadette said that 14 people attended the Hub and all had hospital passports and annual health checks done.

Annual health checks: use the Welsh template (also called the Cardiff tool/template). GPs should be signed up for this.

Most opticians are private. See-Ability can provide some service too.

Information sharing

James is helping with the health facilitation leaflet at his group.

Ros is going on holiday soon. She is going to her brother’s graduation in October.

Sharron said that the summer programme is ready and can be found on line.

New advocacy service derived from the Care Act (since April). Some figures soon said Amanda. Some people referred from Fareham and Gosport.

Supported employment extended contract to May 2016. After this, You Trust is unsure. Some good figures and service is running well providing referred people are suitable for the scheme. Leaflets available.

4 Soo said that Francis is going on holiday with a friend. She also has someone to drive the van at last.

Jenny: My Life My way: 1st Board meeting tomorrow. Jenny is talking to commissioners for supported living (29/07)

Sally said that pathways courses were good.
