Weekly Tuition is to be paid on Thursday PM IN ADVANCE FOR THE FOLLOWING WEEK(s) Fees will be deposited on Thursday night after 6 p.m.

Tuition is To Be Paid: __ Weekly ___ Bi-Weekly ___ Monthly** ___Bi –Monthly*** ** - To be paid no later than the 1st of the month, and is $650.00. *** - To be paid by the 1st & 15th of the month and will be $325.00 per pay day

All Inclusive Tuition Includes: Similac milk based regular Formula, Bottles, Diapers, Wipes, Baby Food & Cereal, Breakfast, Lunch, and Afternoon Snack, Juice and Milk, books, games, craft activities and craft bag, Insect Repellent, Waterproof Sunscreen, Daily reports, Newsletters and toys (inside and out), and Plenty of T.L.C. This contract and financial agreement provides for provider(s) – Paid holidays: New Years Day, Good Friday, Easter Monday, Memorial Day, July 4th – or Friday before or Monday after, Labor Day, Thanksgiving Day AND the Friday after, Christmas Eve, Christmas Day or the Friday before or Monday after and New Years Eve. Plus 8 paid personal / sick days and 3 paid Conference / professional days for providers to attend special daycare related conferences, seminars, classes or meetings.

Provider is also entitled 2 weeks – unpaid vacation days, these days have been deducted when calculating your weekly Tuition. Tuition will be paid in the same amount 52 weeks per year. Days have been preset and are stated in the Policy Handbook.

LATE FEES: see policy book page 2 There is a late fee of $5.00 per 15 minutes, PER CONTRACT that you are late. Late being described as anytime before or after your contracted scheduled hours. This stands at 1-15 minutes = $5.00, 16-30 minutes = $10.00, 31-45 minutes = $15.00 & 46 - 60 minutes = $30.00. There is a $5.00 per day – includes weekends – late fee for payments not made by Thursday Afternoon. AFTER PICK UP Thursday IS CONSIDERED LATE.

Bounced Check Fee is $50.00 PER CONTRACT, PLUS any fees I am charged for any of my checks that may bounce because of your bounced check and late fees from date of that payment.

For childcare to begin, a signed Contract and Financial Agreement, Handbook & Policy Guide, Emergency & Information form, immunization form filled out and signed by physician and the First weeks fees, First and Second Week if paying bi weekly, or amount per days of the starting month and Last 2 weeks prepayment (or prepayment form, for last two weeks ONLY filled out) must be given to provider. Prepaid Holding Fee IS NON-REFUNDABLE. Within 1 month provider will need to have the newborn child’s birth certificate for number.

The Provider reserves the right to terminate this contract immediately at will

This is a valid contract between the two parties that have signed this agreement. All prior terms are subject to change if needed. A one - week written notice will be given in the event changes need to be made. A new contract will be given in the event contract terms change. The fees and any payments owed are not subject to bankruptcy.
