TITLE: Provider Notification

PURPOSE:
To assure nursing staff has a defined sequence by which to notify providers regarding patient issues during clinic hours.
POLICY:
When a patient need requires a provider’s consultation, the nurse shall follow the procedure described below to contact the appropriate provider. On call schedules are distributed monthly.
PROCEDURE:
1. Always attempt to notify the PCP first. 2. If unable to reach or locate the PCP, notify the PCP’s practice partner. Practice partners are: Sharon Carmignani, MD, partners with Deanne White, FNP; Sharon Carmignani, MD, partners with Beth Geden, FNP; and Sharon Carmignani, MD partners with Chris Hartigan, FNP; Andrew Quint, MD, partners with Cynthia Woodcock, FNP;

If unable to reach the practice partner, notify the on-call provider (1st Call). 4. If unable to reach the on-call provider, notify the back-up provider on-call (2nd Call). 5. If unable to reach back-up provider on-call, notify any provider available in the clinic. 6. Document orders/instructions as verbal orders with date and time.
This policy and procedure shall be periodically reviewed and updated consistent with the requirements and standards established by the Board of Directors and by Health Center management, federal and state law and regulations, and applicable accrediting and review organization.
