AC-03 </p><p>TITLE: Provider Notification</p><p>DEPARTMENT: Clinical</p><p>APPROVED BY:</p><p>ORIGINATED BY: Andy Quint, MD; Gloria Crull, CHE</p><p>REVISED BY; Kay Strom</p><p>EFFECTIVE DATE: 11/9/2006</p><p>SUPERCEDES DATE: 10/15/2002, 6/8/2000</p><p>PURPOSE:</p><p>To assure nursing staff has a defined sequence by which to notify providers regarding patient issues during clinic hours.</p><p>POLICY:</p><p>When a patient need requires a provider’s consultation, the nurse shall follow the procedure described below to contact the appropriate provider. On call schedules are distributed monthly.</p><p>PROCEDURE:</p><p>1. Always attempt to notify the PCP first. 2. If unable to reach or locate the PCP, notify the PCP’s practice partner. If unable to reach the practice partner, notify the on-call provider (1st Call). 4. If unable to reach the on-call provider, notify the back-up provider on-call (2nd Call). 5. If unable to reach back-up provider on-call, notify any provider available in the clinic. 6.
