The lender acknowledges that he/she has full authority and power to make this loan, that he/she has read the conditions on the back of this form, and that he/she agrees to be bound by them:

Care and exhibition The University Art Gallery (UAG) will exercise the same care for the borrowed object(s) as it does in the safe keeping of comparable property of its own.
Evidence of damage or loss to the objects will be reported immediately to the Lender.
Unless authorized in writing by the Lender, UAG will not alter, clean, repair or restore the objects.
Insurance Understanding that every reasonable precaution will be taken to insure the safety of the objects while on loan to the University Art Gallery, Indiana State University does not insure objects on loan for student exhibitions.
Reproduction and Credit The Lender grants permission for the objects listed to be photographed, telecast and reproduced for advertising and publicity purposes in both electronic and print media in connection with this exhibition and the University Art Gallery's programs. Any photographs and reproductions of objects shall be limited to publicity and educational use.
Unless otherwise instructed in writing, UAG will give credit to the Lender as specified on the face of this agreement in any publications or exhibition labels.
Sales The University Art Gallery does not handle sales. All inquiries with regard to sales will be directed to the Lender or designated agent.
Change in ownership and/or address It is the responsibility of the Lender or his/her agent to notify the borrower promptly in writing if there is any change in ownership of the objects or if there is a change in the identity or address of the Lender.
Termination Unless otherwise agreed to in writing, the loan shall terminate two weeks after the last day of the exhibition as stated on the face of this agreement.
If the Lender does not pick up the objects within two weeks after the loan termination date, the University Art Gallery, following established museum acquisition or deaccession approval procedures, shall have the right to claim the objects as an unrestricted gift or to dispose of the objects in any manner UAG elects.
