When the ESC receives written notification that you are retiring, we will provide a retirement benefits package which includes Group Life information and Extended Health Benefit options. Employee Name Change

o Group Life Designation of Beneficiary (optional) o PEBA Benefits Plan enrollment (optional) o PEPP Change in Personal Data AND OUR ESC FORM o send copy of mariage/divorce certificate to ESC (if applicable)

If you are out of scope, complete the DIP (Disability Income Plan) form as well.

If you are inscope, it is advisable that you inform your Union.

Termination Q: When will I get my final pay? Check type of employee and pay schedule. If monthly paid, we have a legal requirement to pay within 14 days so if regular pay is not within 14 days, process a Quickpay.

Q: Will my vacation pay, time in lieu be paid out? We will endeavor to pay these items out on your final cheque. Your entitlements will be prorated and the remaining balances will be paid out or recovered on your final cheque.

Q: When do I get my ROE (Record of Employment)?

Your ROE will be included with your final pay package which will be mailed to your home address. Important: Do not wait to receive the paper copy of the ROE before applying for EI (Employment Insurance). An electronic copy will be sent to Service Canada so you may apply immediately. Vacation and Other Accruals Q: I have requested a leave with Income Averaging. How will this leave affect my vacation and sick leave entitlements? Your entitlements will be prorated based on the details of the Income Averaging arrangement.

Q: When am I entitled to Special Northern Leave?

As per the PS/GE CBA, employees who complete one year of service and are entitled to NDA (Northern District Allowance) benefits are entitled to an extra week of Vacation Leave (Special Northern Leave) in addition to their regular vacation entitlement.

The extra week will have been earned at the end of each fiscal year and will be prorated based on the percentage (%) of hours worked.

Q: When do I get my extra week of vacation, i.e., go to the next level of VL entitlement?

Your timekeeper can run a detailed entitlement report showing all hours earned and used to date. If you disagree with any entries on your report you should contact your timekeeper for a review of your timecards.

We do not have your timecards here at the ESC so it is better to work with your local timekeeper.

Q: When do my benefits start? A: For extended health care and dental, you are eligible on the first day of the month following completion of your first six months of employment. o If you have worked the hours equivalent to at least 37.5% of a full time employee, you will receive benefits at this time. o If you have not worked the hours equivalent to at least 37.5% of a full time employee, you will receive benefits when you have.

LTD (Long Term Disability) coverage for Out of Scope and CUPE employees begins after 3 months of service. DIP (Disability Income Plan) coverage for InScope employees begins immediately. Group Life coverage begins immediately for permanent employees. For term employees, your hours are assessed after 4 weeks of employment to determine your eligibility.

Q: When will I receive my GMS card? Will I be reimbursed for claims that occur while waiting for my GMS card to arrive? A: You will receive your GMS card approximately 2 or 3 weeks after your benefits are effective.

You will be reimbursed for claims that are incurred from the date that you are eligible for extended health coverage.

Dental claims are retroactive to your start date. Note: There is no card for Dental coverage.
