Minutes of the Glasgow Obstetrical and Gynaecological Society held in the Royal College of Physicians and Surgeons on 18th January 2012

PRESENTATION

Dr Melrose welcomed members to Trainees’ Night at GOGS and thanked the sponsors for their generosity. She advised the Society that for the first time, the oral presentations will be published in the online version of Scottish Medical Journal.

There were 4 oral presentations and 11 posters presented. All abstracts can be read on GOGS website (except case histories).

Dr Galea: Retrospective audit on use of aspirin in patients at risk of pre-eclampsia In Aug 2010, NICE released an updated guideline on hypertension in pregnancy. Aim of the study – to assess prescribing of aspirin according to the NICE guideline in a DGH. Out of 100 patients, 58 had one or more moderate risk factors and 9 had one or more high risk factors. The audit demonstrated 0% adherence to the NICE guideline ‘Hypertension in Pregnancy’ with respect to the prescribing of aspirin in moderate and high risk patients of pre-eclampsia.

Dr Haworth: An Audit into the Investigation and Management of Women Diagnosed With PCOS At Outpatient Gynaecology Appt The aim of this audit was to assess the diagnosis of PCOS at general gynaecology clinics, the advice given to patients about long term risk of the condition and which treatments were offered, against the RCOG Green Top Guideline 33 Polycystic Ovary Disease, Long Term Consequences. 46 women were given a diagnosis of PCOS prior to, or at this attendance. 24 met the Rotterdam diagnostic criteria. Of the 46 women attending: 19 had their BMI measured, 14 had their blood pressure measured and 4 were offered fasting blood glucose sampling, 13 were advised about the important of regular withdrawal bleeds, 36 were advised regarding the importance of maintaining a normal BMI. This audit shows that we could do more in these areas.

Dr Bowron: A Review of Outpatient Medical Management of Miscarriage at Ayrshire Maternity Unit since its introduction in March 2011 In keeping with RCOG guidelines, in March 2011 outpatient management of miscarriage was introduced in AMU. Patients were given misoprostol and discharged home shortly after. The aim was to reduce in-patient stay without compromising success of procedure. 39 of 52 women having medical management chose rapid discharge. Over 70% of women required no further input from EPAS after their discharge. The percentage of women requiring further surgical evacuation was similar in both groups of patients (8%). This study suggests that outpatient management in this cohort is safe and effective, with the added benefit of a reduction in bed occupancy and staff time

Dr Teoh: Increased perinatal loss in D6 deficient mice D6, is highly expressed by the placenta and has been shown to protect against fetal death induced by bacterial endotoxin or anti-phospholipid, and aids fetal survival after embryo transfer into allogeneic recipients. The impact of D6 deletion on perinatal outcomes of naturally conceived syngeneic fetuses in unchallenged DBA/1 mice, postnatal growth, d14 and d18 placental stereology was assessed. The lack of D6 caused a highly significant increase in the frequency of stillbirth and neonatal death in first 3 wks after birth of D6 deficient pups. The D6 receptor is critical for normal Labyrinthine Zone volume development and fetal growth, however, late compensatory mechanisms exist to ensure normal fetal weight at term. This initial abnormal placental development may underlie the observed increase in stillbirth and neonatal deaths.

All speakers received a number of questions from the audience after which Drs Mathers, Jarvie and Hair retired to judge the posters and oral presentations. During this, the AGM was held (see minutes of AGM).

Award of prizes to Trainees and Students – Dr Mathers congratulated all of the oral and poster presenters for the amount of work put into each presentation, and gave some words of wisdom including a reminder about the value of spell checkers, and correct use of the word ‘audit’. Dr Melrose presented the following with their medals. The student prize was divided between Angela Gillan: Management of women with perceived reduced fetal movement and Catriona Kerr-Wilson: Prevention of Mother to Child Transmission of HIV in Zimbabwe The poster prize was awarded to Dr C Neill: Labial reduction surgery in a district general hospital from 2007 to 2010 The Matt Carty Award for the best oral presentation was awarded to FY2 Dr Carly Bowron: A Review of Outpatient Medical Management of Miscarriage at Ayrshire Maternity Unit since its introduction in March 2011

Dr Melrose reminded the society that Dr Mary Hepburn will speak on ‘health inequalities, an evolving challenge for maternity services’ at the next meeting on 22nd February, 2012.
