Sample Parent Letter: Explicit Consent Model Retesting Your Student Who Has Met or Exceeded the State Achievement Standard

Dear Parent,

As of January 2012, the Oregon Department of Education no longer permits school districts to re-test students in grades 3 – 8 who have already met or exceeded the state achievement standard on the statewide Oregon Assessment of Knowledge and Skills (OAKS) unless the student’s parent or guardian permits the school district to retest their child.

Your child took the OAKS in subject on date and met/exceeded the state achievement standard. For purposes of insert rationale , we would like to retest your child. The retest is scheduled to occur on date . If you permit for your child to be retested, please sign and return the bottom half of this letter to return information by date .
