Proctor Directions Grade 5 Math Summative Assessment Fall 2014

 This is a secured test.

 Per the test security agreement: I (the teacher) will not view, print, copy, and/or duplicate any part of the test (unless explicitly directed by the district test coordinator to do so). Violations of the test security agreement will result in disciplinary action.

 Proctor directions may be printed.

 All students will take their grade level assessment.

 All testing and make-ups must be completed within the Schoolnet testing window. (October 20-November 7, 2014).

 The ONLINE PASSCODE is 5MXQWP.

Notes to the teacher: Be sure to read the following at least a day before testing.

 Please review all directions carefully before you begin discussing the assessment with students.

 The assessment aligns to the 2010 Arizona College and Career Ready Standards.

 The assessment will be administered by teachers and trained staff. Scoring will be done automatically in Schoolnet.

 Students must complete the assessment online in Schoolnet in a computer lab unless pre-arrangements have been made with your school administrator. Therefore, students will need to know their username and password (it is the same for logging on to the Kyrene system as well as logging in to Schoolnet).

 All students must complete the assessment independently.

 The test is not timed. However, the test should take about 60 minutes. Students should be able to complete the assessment in one sitting and according to the timeline your school has established. Allow time for giving directions. If a student goes over the time allotted, Schoolnet will not shut the student out and the student should continue taking the assessment.  If you experience technical difficulties, please contact tech support at extension 1215.

 If a student has extenuating circumstances (for example, a student is sick and goes home) and needs to complete the test on another day, please fill out the form available on the Kyrene Summative Teacher Directions webpage (you must be signed in to the website), click on Resuming/Deleting Student Assessment Request. Otherwise, the test should be finished within the testing period established by your school.  Students must log off the computer after submitting the assessment. Please ask students to bring a book to read for when they are finished or have an independent activity ready for them.

 Teachers are responsible for actively monitoring during the test, but cannot help students with the problems. Accommodations outlined in IEPs and 504s should be followed.

 The assessment should be taken in one session without breaks.

Distribute student scratch paper and pencils. Students will need to know their username and password for logging on to the system and Schoolnet. In addition, teachers will need to provide the online passcode to students.

Part 1: Session 1

Step 1: Introducing the Assessment (Teacher Directed)

Read the following information verbatim to students: Today, you will be taking the Math portion of the District Summative Assessment. There are 20 questions on the test. Talking during the test is not allowed.

All your work will be done in the computer lab and completed independently. You will have scratch paper to work out problems. Calculators may not be used.

Step 2: Logging on to the Computer and Schoolnet

Say: Now we will begin the Math Summative Assessment. Log on to the computer using your Kyrene username and password. Next, click on the Internet Explorer icon. You should see the Kyrene School District Website. Select Students > Educational Resources > Schoolnet. Login using your Kyrene username and password. Enter the online passcode (see directions above for finding the online passcode), click Go, and then click Start Your Test. Do not click Start Test Now until I finish giving the directions.

Say:

Please use the scratch paper provided to show your work. Work every problem carefully.

When we are finished testing wait for me to come to your computer. (Teacher should at this point go over and check that every question has been answered. Then, allow the student to click the Submit test for Grading button.) Once you have clicked on the Submit test for Grading, please log off Schoolnet and the computer.
