Brief Account of Incident: Finger was pinched while trying to close a door. The employee was entering the Pump room door from the Operators cab on a Pump unit. As the employee was closing the door, he was distracted by another individual calling for his attention. The employee lost focus of his hand placement as the door continued to close, pinching his Ring finger. The employee was taken to the MCC Medic and diagnosed with an open fracture / laceration.
Potential Outcome: DAFWC
Investigation findings: Lack of focus or attention caused by distraction.
Hazard ID/Mitigation Adequacy Company wide Pinch Point Hazard recognition program and presentations were implemented this year, previous to incident. This pinch point injury was the unnatural result of pushing away the door to close with the hand around the outside edge of the door above the handle. The employee was immediately dispatched to Medic for care, and notifications passed on to appropriate parties.
What Went Wrong Possible Immediate Cause; 4-Lack of focus or attention 4.1-Distraction by other concerns.
Possible System Cause; 18- Engineering / Design

Corrective Actions: Discuss incident at weekly HSE Safety meetings. Engineer and install safety latch guard.
