<p> Bp Exploration (Alaska) Inc. safety Safety Communication Lessons Learned sharingtheexperience</p><p>Type of Incident: Recordable Injury Traction Number: 2008-IR-2905243 Asset: Greater Prudhoe Bay Location: Drill Site 5-15 Date: 12-28-2008 </p><p>Brief Account of Incident: Finger was pinched while trying to close a door. The employee was entering the Pump room door from the Operators cab on a Pump unit. As the employee was closing the door, he was distracted by another individual calling for his attention. The employee lost focus of his hand placement as the door continued to close, pinching his Ring finger. The employee was taken to the MCC Medic and diagnosed with an open fracture / laceration.</p><p>Potential Outcome: DAFWC </p><p>Investigation findings: Lack of focus or attention caused by distraction. </p><p>Hazard ID/Mitigation Adequacy Company wide Pinch Point Hazard recognition program and presentations were implemented this year, previous to incident. This pinch point injury was the unnatural result of pushing away the door to close with the hand around the outside edge of the door above the handle.
