3. Education: List degrees, academic status - class, G.P.A., major, extra-curricular activities.

4. Statement of 200-800 words describing the activities and accomplishments of the candidate that led to "the advancement of the interaction of the profession and the humanities”, specifically work and interest in the arts, foreign languages, English, history, linguistics, music, philosophy, religion, or theater.

5. Statement of 50-200 words relating the personal character of the candidate and his/her influence for the enhancement of the profession, the good of the profession and the good of his/her community and fellow man."
