*____ (Initial) Applicant agrees to pay $600 deposit upon registration and CCEC will purchase the air ticket and fully refund the deposit within three days of applicant arriving and registering in China. Air ticket deposit of $600 will not be refunded if the applicant cancels the trip for any reason after April 1, 2016

**____(Initial) CCEC will automatically enroll you with INEXT International Travel Insurance. This is optional, however; if you choose to opt out you will need to provide proof of international travel coverage (please see waiver).

****NOTE: IF you do NOT complete the program once in China, you will be responsible for rescheduling your return ticket and pay the difference plus any fees accrued.****

Complete this form; and submit to CCEC (via email). Print a copy of the form containing your signature Mail copy with the following: teaching certificate, two recent passport photos, and copy of the first page of passport. Please contact CCEC to arrange payment schedule, if needed.

Chinese Visa information will be emailed to participants after registration deadline. If you do NOT have a passport, we strongly recommend you apply for one upon submitting your registration form. You will be advised when to submit visa application and passport.

PAYMENT METHOD: Check or Money Order, Cashier Check, wire transfer or bank deposit, must be in U.S. funds payable to: CCEC. There will be a $50.00 fee charged on checks returned by the bank due to insufficient funds. Registration confirmation/receipt and further information will be emailed.

All applicants are subject to a background check to be eligible.

****Note: Please check with your school district to see if you qualify for 60 credit points toward career development/recertification by participating in either above program.
