Lassiter/Pope/Kell NJROTC 2017/2018 CPO Registration and Payment Agreement

As the parent and/or legal guardian of ______(cadet name), our signature below indicates that we have been advised and informed of the following:

• We have been advised of and understand that the there is often a cost of participation in Lassiter/Pope/Kell NJROTC Events. We understand that we are responsible for the amounts incurred.

• We understand that the Unit will attempt to make fund raising activities available to us, and if we are successful, 70% of the amounts raised will be applied to the Cadet Accounts of the Cadets who participate in the fundraiser (which is used to help offset the individual cost of participating in competitions) and 30% will be applied to the CPO General Fund to cover administrative costs of the CPO. However, if we are not successful or my cadet does not participate, and/or my Cadet’s Account lacks sufficient funds to participate in any and all events, we understand that we must pay all amounts due in full, and further understand that the Unit CPO will not offset these expenses.

• If the Cadet named above elects to participate in any of our competition teams, we understand that any balance in excess of the amounts described below may be applied to other Unit Activities, but that we may not apply Cadet Account funds to other activities unless the sum described below is satisfied.

Our signature below indicates that we understand and agree to the policies outlined in this Agreement and that upon request we will receive a copy of this Agreement for our records.
