JOB DESCRIPTION

TITLE: Care Coordinator

REPORTS TO: Lead Care Coordinator

CLASSIFICATION: Salaried, exempt

GENERAL DESCRIPTION: Serve as a member of an Interdisciplinary Team for YESS’ Integrated Health Home Department and play an active role in designing an integrated care plan for children and their families. As the first point of contact for clients, Care Coordinators are primarily responsible for completing assessments, making and tracking referrals, following up on referrals, and coordinating with internal team members and other professionals.

QUALIFICATIONS: . Bachelors Degree in Social Work or related field. . Experience working with children and families in a community-based program preferred. . Knowledge of community resources. . Demonstrated ability to work effectively with team members. . Excellent written and verbal communication skills. . Computer knowledge.

DUTIES SPECIFIC TO THE POSITION: . Schedule and track referrals and appointments. . Follow-up on referrals and appointments. . Communicate with providers regarding client’s progress. . Promote client’s health and ensure client’s goals are developed and documented. . Coordinate multiple systems for clients and their families. . Provide education and resources to clients and their families. . Participate in wraparound planning processes. . Engage clients and families eligible to participate in Integrated Health Home Services. . Identify community resources and provide ongoing support clients and their families. . Facilitate development of crisis and other client and family-driven plans. . All other duties as assigned.
