Filing Fee form To qualify for reapproval using this form, the expired plat SHALL 10 copies, 1 133R NOT BE CHANGED from the previously approved version, unless with a signed “Contents of Plat Drawing (Original) otherwise specifically allowed or required by the OMPC.
