OREGON CONSOLIDATED DISTRICT PERFORMANCE REPORT TITLE I-D Subpart 2 School Year 2006 -2007 Due to technical difficulties the online Consolidated District Performance Report survey does not include Title ID subpart 2. The following Title ID subpart 2 CDPR document will be used in place of the online survey to complete the Title ID subpart 2 portion of the CDPR required by USDE. Title I, Part D, Subpart 2
General Data Reporting Form The tables contain annual performance report requirements for the Title I, Part D, Subpart 2, Neglected or Delinquent Education Program for 2006-07 School Year, defined as July 1, 2006, through June 30, 2007.
Instructions: District Title I, Part D, Facilities and Students
If the district is contracting educational services from more than one facility include information on all facilities. If the district is using Title ID subpart 2 funding to provide a program for ‘at-risk- students’ within the district report those students in “at-risk-student” column. If the district contracted services and provided an “at-risk-student” program report on both.
In the first column, report the facilities and or programs served with Title I, Part D, Subpart 2 funding. List the names of the facilities and programs by type, including at-risk programs, neglected programs, juvenile correction facilities and juveniles in adult corrections facilities. Juveniles in adult correction facilities, eligible for Title ID subpart 2 assistance, must: Be eligible for a Free, Appropriate Public Education The academic program must be: o provided by the district, using district resources, Title ID subpart 2 is a supplemental program o of at least 20 hours per week of academic instruction o comparable with the local district program o aligned with Oregon Content and Achievement Standards
In the second column, indicate the duplicated number of at-risk, neglected, or delinquent students who were admitted to each type of program/facility. A duplicated count is one that counts students each time they were admitted to a facility or program during the reporting year. In the third column, enter the average length of stay (in days) for students in each type of facility/program. The average should include multiple visits for students who entered a facility or program more than once during the reporting year.
In the fourth column, indicate the unduplicated number of students admitted to each type of facility/program. An unduplicated count is one that counts students only once, no matter how many times they were admitted to a facility or program, within the reporting year.

However, students may be counted in more than one outcome category within the same enrollment period (e.g., returned to school and earned high school credits). Section 2.1 For Section 2.1 of this table, items 1 and 2, enter the number of students who attained the following academic outcomes during their time in the facility/program: earned high school course credits and/or were enrolled in a GED program. Report the numbers by program type (e.g., Neglected Programs or Juvenile Corrections.) For Academic & Vocational Outcomes, if the district has more than one facility serving neglected students, combine student data from all facilities for neglected students. If the district has more than one facility serving delinquent students, combine student data from all facilities for delinquent students. If less than 6 students were served in any category use an asterisk (*) in the appropriate cell(s) and add a note at the end of the table. 2. Academic Performance Tables: Reading and Math Instructions: Academic Performance Tables Report the number of long-term Title I, Part D, Subpart 2 students in neglected programs or juvenile corrections who participated in pre- and post-testing in reading and math. Long-term refers to students who were incarcerated for at least 90 consecutive calendar days from July 1, 2006, to June 30, 2007.
The reported numbers should represent unduplicated counts of students; report only information on a student’s most recent testing data. Count each student in only one length of stay category. For each length of stay category, report the data by the following facility or program type: students in neglected programs (N) and students in juvenile corrections (JC).
For item 1, enter the number of students who were in placement during the reporting year for either 90-179 days, 180-270 days, or more than 270 days, by type of facility/ program.
For item 2, enter the number of students reported in item 1 who tested below grade level when they entered the facility or program.
For item 3, enter the number of students reported in item 1 who have data available for both the pre and the post test exams.
For items 4-8, indicate the number of students reported in item 3 who showed either negative change, no change, up to ½ grade level change, up to one grade level change, or more than one grade level change on the pre-post test exam. Students reported in item 3 should not appear in more the one of these change categories. The sum of the numbers reported under each category , in each table, in items 4-8 should = the total of each category in item 3.
