Job Title: Care Manager - Residential Reports to: Group Home Manager Department: Residential Department  Exempt x Non-Exempt Revision Date: December 2016 Titles of Direct Reports: None

JOB PURPOSE The Care Manager provides the oversight of the clients care while residing in the group home. They provide the linkage between Gwen’s Girls, the client, her family, the placing agency and other providers ensuring the client’s needs are being met. The Care Manager works cooperatively with the client and their family to develop and implement an individualized service plan designed to help clients build targeted skills and reach established goals.

MINIMUM REQUIREMENTS Education: A Bachelor Degree in related field.

Licensure/Certification: N/A

Minimum Requirements:  Minimum age requirement is 21 years old.  Demonstrated 2+ years’ experience in residential care or case management.  Strong interpersonal relationship and communication skills necessary.  Ownership of a reliable vehicle; a valid driver’s license, and proof of insurance required in the minimum amount of 300/100. Proof of these requirements must be presented upon hiring.

Clearances Required: PA Act 33 (Child Abuse) / Act 34 (Criminal History) / Federal Criminal History Record (FBI Fingerprint)

Other Skills/Abilities:  Ability to communicate verbally and in writing to ensure full compliance with agency and government regulations.  Ability to be a team player working and communicating effectively with the Therapist, Program Staff and other Service Providers Ability to employ passive physical restraint techniques in crisis situations  Ability to drive a high occupancy vehicle or own vehicle with legal proof of driver’s license, and insurance.  Ability to demonstrate effective leadership, organization, and time management skill.  Ability to make safe and appropriate decisions during times of crisis

POSITION SUMMARY  Promote the philosophy and mission of Gwen’s Girls in the provision of all responsibilities, displaying an understanding of the basic philosophies and principles of gender-specific practices and working with clients who have experienced trauma.  Establishes a caring, nurturing relationship with each assigned resident and partners with the resident’s family through ongoing face-to-face contacts. Actively contacts families of residents to improve communication and relationships and to facilitate reunification when applicable. Develops and implements individualized service plans (pathway plan) to include goals, skill-building activities, and measurement of set goals. Reviews and reassesses service plans at least quarterly on each client. Participates in Conferencing and teaming meetings within the home, as necessary. Provides proper documentation of all contacts. Documents and reports progress, concerns, etc. Communicates regularly with all treatment team members to keep properly informed.  Presents reports at team meetings, quarterly staffing, and transfer conference. Coordinates/participates 30-Day ISP, Monthly Reports, Transfer, and Discharge Meetings. Meets regularly with program manager and keeps properly informed of concerns, progress, etc. regarding residents.

Page 1 of 2  Completes all required documentation and reports including service plans, safety assessments, home visit logs, monthly reports, statistics and data needed for program accountability and billing into Key Information Demographic System (KIDS) in a timely manner according to established standards and deadlines.  Serves as liaison with placing agency and with external service providers. Ensures that all active parties are working collectively to reach the client goals. Maintains ongoing communication (no less than once a month) with placing agency, school and all other outside resources on behalf of Gwen’s Girls and our clients.  Actively participates in all required team meetings, trainings and supervisory sessions. Meets regularly with supervisor, communicates openly needs, concerns, and ideas; utilizes feedback given in supervisory sessions.

 Performs in accordance with established system-wide competencies and receives satisfactory performance evaluations annually.  Performs other duties as assigned.

WORK ENVIRONMENT Works in a group home setting with adolescent girls with adverse backgrounds. Staff must be prepared to engage in verbal and physical interventions in the event of a crisis.

Physical Requirements: Sitting, standing, kneeling, bending, stooping, reaching over head, speaking, hearing, reading comprehension and the ability to lift and carry 50 pounds.

General Requirements: 1) Indoors in busy office, working in close proximity to co-workers; 2) Frequently required to work at a fast pace; 3) Requires considerable concentration and creativity; 4) Some weekend and evening work required, if necessary; 5) Travel required (transporting clients).
