PROTOCOL FOR INSTITUTIONAL INVESTIGATIONS IN CLAY COUNTY

(Note: The following protocol should be followed on all Child Care and Foster Care Licensure institutional reports)

RECEIPT OF REPORT:

1. Report alleging abuse, neglect or abandonment in a Foster Home, Group Care Facility, Child Day Care Facility, Family Day Care Home or Large Family Child Care Home is received in the Report Assignment Unit.

2. Report Assignment Unit will assign report to CPI.

3. Upon receipt of the report, CPI will telephonically notify the appropriate licensing representative to advise of the report and coordinate investigation. CPI will also send email notification to the Foster Care/Group Care licensing agents below with Report #, name of facility or foster parent, their name and contact information.

If the licensing representative is not available and a voice mail message is left, the CPI should clearly communicate their name, phone number and the CSA #.

4. CPI and Licensure Counselor will make every effort to conduct joint investigation. 5. CPI will link provider to the CSA within 5 days of receipt of the report.

Incident Reports Please complete an Incident Report when any of the following occur:

1. Child Deaths 2. All Media cases or anything that may cause media attention 3. Arrests of parents and/or caregiver responsible (ie: relative/non-relative/foster or group home parent) when the arrest is related to child abuse/neglect 4. Sexual abuse cases involving foster homes or group homes 5. Reports on foster homes where we have concerns or there are verified findings

INSTITUTIONAL STAFFINGS:

1. There will be “standing” institutional staffing (s) the second and fourth Tuesday of each month. If a staffing needs to be scheduled, contact Latonjia Trice, Administrative Secretary, Child Care Licensure Office, at 723-2064. Child Care staffings will be held in room 375, 3rd floor of the Roberts Building and the Foster Care staffings will be held in the Family Safety Conference Room (Room 219).

2. If allegations of institutional report might result in findings of “some indicators or verified” a staffing must be scheduled by the CPI within 14 days of receipt of report. The CPI will notify the respective licensing counselor and they will jointly determine other parties to invite to the staffing.

3. A staffing may also be scheduled when there is a finding of “no indicators” if the assigned CPI or Licensure Representative has concerns about the investigation or history of the home or facility.

4. The CPI, CPI Supervisor, CPI Program Administrator, Licensing Counselor, Licensing Supervisor and Licensing Program Administrator are required to attend the Institutional Staffing.

5. An Institutional Staffing Review Form will be completed at the staffing by the chairperson or designee and signed by all participants of the staffing. Copies will be placed in the licensing and CPI files.
