This agreement for usage of the facility is issued to the above mentioned Name/Organization (Lessee) on the date and for the specified purpose. The lessee of the facility will comply with the agreement and will be held responsible for any damages resulting from or during usage of the facility. The lessee will be responsible for cleaning the facility prior to leaving the building. Rental fee, Kitchen fee, and waste disposal fee (if applicable) mentioned above will be paid by the user at the end of the event. A deposit of $30.00 (money order) is requested prior to the rental and day of usage.

1. Appoint a Security Person whom will keep the outside and inside facility safe and free of vandalism during the ENTIRE event. Name:______2. Children are not allowed at Bingo activities. 3. Children must be watched at all times. 4. Clean the lobby and kitchen area, BOTH restrooms, sweep and mop. 5. Remove all decorations, staples, tape, and thumb tacks. 6. Please provide your own trash bags and take your trash with you or fees will be imposed. 7. All tables must be covered & covers removed at end of usage. 8. Put all chairs and tables away properly and make sure they are cleaned. 9. Turn off all lights in the building. 10. Facility must be free of alcohol and drug usage. 11. Chapter Administration will CANCEL/POSTPONE usage at own discretion, due to a number of family gathering for Funerals. Sponsor will be given another date of usage. 12. In the event of a Bingo cancellation; a fee of $20 will be imposed when lessee uses up to more than 2 hours of usage. I have read the above agreement and agree to abide by all rules and understand that I represent the person/organization (lessee) with the responsibility for using the facility. I understand the deposit will be refunded if all areas are cleaned, in working order, and all other items mentioned above are followed. I also understand in the event I am in non-compliance, I will not be able to use the chapter for a 3-6 month interval and the deposit will be forfeited PLUS any fees imposed.
