2017 Scholarship Application

The Wisconsin Alumni Association: Twin Cities Chapter is offering scholarships for current high school seniors who will be pursuing a degree at the University of Wisconsin-Madison beginning in the fall of 2017. Students must be accepted to the UW-Madison and currently attend a high school in the 7-county Twin Cities metro area (Anoka, Dakota, Carver, Hennepin, Ramsey, Scott and Washington counties). Scholarship awards are paid directly to the university and applied towards freshman year tuition. The amount of each scholarship awarded may vary. Completed applications must be received by 11:59PM Tuesday, February 28th, 2017.

Your completed application will include the following documents:

_____ Two letters of recommendation from someone other than family or friends (i.e., employer, high school faculty, church leader, etc.)

Why do we ask for Campus ID and FERPA forms? In the event of being awarded a scholarship, your Campus ID is needed to link your student account to the scholarship payments. Additionally, FERPA information helps establish a level of financial need. Even if you think you don't have a financial need, we would encourage you to submit the FERPA form.

Additionally, if you are selected to receive a scholarship, we will require the following during/following your freshman year at the University of Wisconsin-Madison:

1. Your on-campus mailing address and email address (no later than October 6th, 2017).

2. One letter to the WAA: Twin Cities Chapter describing your first semester on campus (by December 31, 2017).

3. Attendance at one of the following events:

• Annual Founders’ Day Dinner (Spring 2017), or

• Senior Send-Off Picnic (late July/early August 2017) Application Questions

Please provide a comprehensive answer to each of the following questions. You may add additional space/ pages as necessary.

1. List and describe, as necessary, your high school extracurricular activities (i.e., clubs, yearbook, newspaper, drama, sports, etc.). Include the amount of time you spend on each activity.

2. List and describe, as necessary, activities outside of school (i.e., community, volunteer, church, synagogue, etc.). Include the amount of time you spend on each activity.

3. List your hobbies/interests.

4. Do you have a job outside of school? If so, please describe it and include your hours/week.

5. How did you find out about our scholarship? (if “online” please list the specific website)

6. How do you plan to finance your education and related expenses? (Indicate % from work, savings, scholarships, family, etc.)

7. In 150 words or less, describe your goals and course of study at the University of Wisconsin-Madison, your intended major, and your expectations of the University of Wisconsin-Madison.

You will be contacted regardless of whether you have been selected for an interview. Late or incomplete applications will not be accepted.
