FACULTY MEMBER PREPARATION FOR ANNUAL MENTORING COMMITTEE MEETING

1. Update your CV using the UWSMPH format available under “appointments, reappointments and promotions” on the HR site of the School of Medicine intranet http://intranet.med.wisc.edu/

2. Outline recent academic accomplishments (since last meeting) using the following list of teaching, research, clinical, and administrative activities

------Teaching:  Course name, department, approximate numbers of hours and students.  Development of new courses or interdisciplinary courses  Mentor teaching: postdoctoral fellows, graduate and undergraduate students, medical students, residents and fellows  Medical student clerkships and clinical teaching of residents, demonstration, one- on-one teaching in a clinical setting  Lectures to medical students, residents, referring physicians  Development of new programs and national, regional or local dissemination of new knowledge (seminars, CME programs, etc.) ------Research: Publications:  Articles published in peer-reviewed journals  Book and book chapters  Other non-refereed publications  Articles in press or submitted (do not list manuscripts in preparation) Grants: For all grant activities provide title, amount, source of funds and dates, contribution to project (e.g., PI, Co-I).  Active ongoing funding  Newly funded grants  Grants submitted but pending or not funded (give percentile/priority score)  Intramural or other local funding Presentations at meetings: Include place, date and title.  Visiting professorships and invited lectureships at other universities, postgraduate courses, national and international meetings  Abstract presentations at national or international meetings (poster, oral session presentation)  Invited speaker at regional or state meetings  Invited lectures at UW ------Clinical activities:  Activities in area of clinical expertise, such as inpatient and outpatient clinical care  Maintenance of expertise in area  Contributions to advancement/innovations in field, evidence of a scholarly approach to patient care ------Administrative/Leadership:  Administrative/service activities: include time commitment, duties and responsibilities on committees or boards  Departmental committee assignments and administrative duties. Hospital, Medical school and UWMF committees  UW committee assignments and administrative duties.  Invited service on national boards, e.g., NIH study section, ABOG examiner, officer or committee member of professional organizations  Other professional, scholarly or service activities not included above  Other noteworthy accomplishments: UW, regional or national activities that enhance Department stature and positive image ------3.
