SERVICE DEFINITIONS

The following information is provided as a reference in the preparation of the proposal. The Area Agency on Aging is seeking proposals in the following areas.

A. Program Name: ADULT DAY CARE

1. Program Goal: To enhance the well being and improve the lives of older persons living with dementia and other chronic conditions.

2. Program Definition: ADC centers are licensed community-based programs that provide non-medical social day care services to older persons 60 years of age or older.

2. Required Units of Service (M.I.S. definition):

Hours of Attendance: To provide an hour of attendance for an eligible participant at a facility I center.

4. General requirements  Day care for impaired older persons 60 year of age and older, including a supervised, organized program of social, recreational, and nutritional services.

B. Service Name: ALZHEIMER’S DAY CARE RESOURCE CENTERS (ADCRC)

1. Program Goal: To enhance the well being and improve functioning of adults and older persons with dementia.

2. Program Definition: Alzheimer’s day care services and support

3. Required Units of Service (M.I.S. definitions): Alzheimer’s Day Care Participant Days: To provide a supervised and organized program of social, recreational, nutritional, and education services for individuals with Alzheimer’s Disease or a related disorder, to enhance their well-being and functioning. Unit: Day

Alzheimer’s Support Group: To provide monthly caregiver support group services to family members of program participants and other community members. Unit: Session

Counseling: To provide emotional support to family members of program participants. Unit: One hour

Community Education: To provide information and education regarding Alzheimer’s disease, related dementias, care giving skills and other related areas. Unit: One session

Alzheimer’s Volunteers: Number of volunteers in the contract period Unit: Count

5. General requirements:  Program shall provide day care services to persons living with dementia and their caregivers  Duties: 1) Provide adult day care for persons suffering from Alzheimer’s disease, related dementia and other frail elderly afflictions 2) Provide increased socialization, assistance in achieving and maintaining the highest possible level of physical/mental functioning and independence 3) Provide respite for caregivers 4) Provide support group services for caregivers 5) Provide community information and education services 6) Provide counseling services to families of program participants 7) Recruit, train and monitor services of volunteers

C. Service Name: CONGREGATE NUTRITION

Components:  Congregate Meals (Central Kitchen and Caterers)  Senior Dining Centers

1. Program Goal: To maintain or improve the physical and social well being of older persons through appropriate nutrition services.

2. Program Definition: Provision of a meal to an eligible client or other eligible participant at a nutrition site, senior center or some other congregate setting.

3. Required units of service (M.I.S. definition)

A. Component Area: Central Kitchen/Caterer – Unit of Service Definition

Congregate Meals (1 Meal) – Provision to an eligible client or other eligible participant of a meal at a nutrition site or senior center of a meal, at minimum 5 meals per week

Meals comply with the 2005 Dietary Guidelines for Americans (published by the Secretary of the Department of Health and Human Services and the Secretary of the United State Department of Agriculture)

Provides, a minimum of 33 and 1/3 percent of the current daily Recommended Dietary Allowances (RDA) as established by the Food and Nutrition Board of the National Research Council of the National Academy of Sciences . Page 2 of 16 ATTACHMENT F, RFP 9879, Area Agency on Aging

B. Component Area: Nutrition Education - Unit of Service Definition

(121) Nutrition Education (1 session) – Provision of nutrition information and education to an eligible client at a nutrition site or senior center

5. General Requirements:  Each Elderly Nutrition Program provider shall establish and administer nutrition services with the advice of a registered dietitian.  Each Elderly Nutrition Program shall provide at least one (1) meal per day.  Meals shall be served at least five (5) days per week throughout the service area, but not necessarily five (5) day per week at each site.  Each Elderly Nutrition Program may operate at a lesser frequency in a service area where such frequency is not feasible and the AAA approves a lesser frequency.  Must comply with the California Uniform Retail Food Facilities Law (CURFFL) and local health department regarding safe and sanitary preparation and service of meals.  Comply with the Division of Occupational Safety and Health (Cal/OSHA), California Department of Industrial Relations requirements regarding staff and participant safety.  At a minimum, perform quarterly monitoring for safe food handling and sanitation practices of food facilities.  Conduct a nutrition screening of congregate and home-delivered meal participants in accordance with Federal requirements.

D. Service Name: FAMILY CAREGIVER SUPPORT

1. Program Goal: To promote the well being of family caregivers of older persons 60 years and older.

2. Program Definition: To support and train families at all stages of caregiving.

SERVICE INFORMATION

 Outreach: Activities initiated by an agency or organization for the purpose of identifying potential caregivers and encouraging their use of the existing services and benefits. Unit: One on one contact with a family caregiver

 Community Education: To educate groups of caregivers about available services and supports. Unit: One hour

ACCESS

 Information and Assistance: Activities that (A) provides the individuals with resource information; (B) Screens to identify caregiver service needs; (C) Refers caregivers to appropriate services; (D) To the extent feasible, performs follow-up to ensure services received. Unit: One contact  Case Management/Assessment: To provide ongoing caregiver support through assessment, planning, service arrangement and monitoring. Unit: One hour  Transportation: Assisted or unassisted, to provide a means for caregivers to go from one location to another. Unit: One way trip

CAREGIVER SUPPORT

 Counseling: To provide guidance and emotional support for family caregivers. Unit: One hour  Support Group: A group of caregivers led by a facilitator for the exchange of information and delivery of emotional support. Unit: One hour  Caregiver training: To provide group and individual instruction and education on caregiver skills training. Unit: One hour

1. Program Goal: To prevent disease and increase the health awareness of persons 60 years of age or older.

2. Program Definition: To deliver individual and group health promotion and disease prevention training and education.

3 Required Units of Service (M.I.S. definition):

Disease Prevention and Health Promotion: provision of community information and education to persons 60 years of age and older. UNIT: 1 session

4. Area to be Served: Monterey County

5. General requirements  Disease prevention and health promotion activities may include, but not be limited to disease prevention, health promotion and exercise and nutrition education.

F. Service Name: HEALTH INSURANCE COUNSELING AND ADVOCACY PROGRAM (HICAP)

1. Program Goal: To improve knowledge of older persons regarding health insurance issues.

2. Program Definition: To provide health insurance information and educational services to older persons and to advocate on their behalf through counseling services.

A. HICAP—Fund/Reimbursements

M.I.S. Unit of Service Definitions:

Counseling : To provide objective information about health insurance policies and issues. UNIT: one hour

Education/Training: To provide educational services to the public on health insurance counseling and advocacy issues. UNIT: one session

Legal Services: To provide legal counsel and representation for issues related to insurance policies, issues or claims. UNIT: one session

B. HICAP—SHIP

C. M.I.S. Unit of Service Definitions:

SHIP: To support the activities of the Fund/Reimbursement Program.

4. Area to be served: Monterey County

5. General requirements (Fund/Reimbursement Program):

* To provide health insurance counseling and advocacy services to the public and Medicare beneficiaries. Services shall be provided throughout the County of Monterey.

Services include: 1. Health insurance counseling 2. Health insurance education and training 3. Health insurance related legal services 4. Recruitment, training and supervision of volunteer counselors

General requirements (Medicare Modernization Act(MMA) Program):

Services include: 1. Increasing recruitment, training and supervision of volunteer counselors 2. Expanding counseling locations in areas near low-income and hard to reach beneficiaries

3. Activities that support implementation of the Medicare Modernization Act of 2003 (MMA)

G. Service Name: HOME-DELIVERED MEALS

2. 1. Program Goal: To maintain or improve the physical and social well being of older persons through appropriate nutrition services. Provision of a home delivered meal to an eligible client or other eligible participant.

2. Required Units of Service (M.I.S. definition):

Program Area: Home Delivered Meals

Home delivered Meal (1 Meal) – Provision to an eligible client or other eligible participant at the client’s place of residence, a meal that:

Complies with the Dietary Guidelines for Americans (published by the Secretary of the Department of Health and Human Services and the Secretary of the United States Department of Agriculture);

Provides, if one meal is served, a minimum of 33 and 1/3 percent of the current daily Recommended Dietary Allowances (RDA) as established by the Food and Nutrition Board of the National Research Council of the National Academy of Sciences.

Provides, if two meals are delivered together a minimum of 66 and 2/3 percent of the current daily RDA.

Program Area: Nutrition Education

Unit of Service Definition

Nutrition Education (1 session) – Provision of nutrition information and education to an eligible client at the client’s place of residence. Services may be delivered in person or via printed material

3. Area to be Served: Monterey County

Present geographical subdivisions include:

Monterey Peninsula: Marina, Seaside, Monterey, Pacific Grove, Carmel, Carmel Valley, and Big Sur Rural: Southern Monterey County and Northern Monterey County Salinas: City of Salinas

4. General Requirements for home delivered meals:  Each Elderly Nutrition Program provider shall establish and administer nutrition services with the advice of a registered dietitian.  Each Elderly Nutrition Program shall provide at least one (1) meal per day.

H. Service Name: INFORMATION AND ASSISTANCE (I&A)

1. Program Goal: To enhance consumer awareness and knowledge about community resources for seniors.

2. Program Definition: A service for older persons that provides: (1) current information on opportunities and services; (2) screening and identification of consumer service needs; (3) linkage of consumers to opportunities and services; and, (4) follow-up on referrals with the client or organization to ensure services referred met the clients need.

4. 3. Required Units of Service (M.I.S. definition):

Unit of Service definitions:

Information: To provide the community with information about senior resources, programs and services (Does not attempt to facilitate appointments or other arrangements between the client and service provider.) UNIT: one client served

Referral: To screen inquirers to identify needs and appropriate services and resources; assist in identifying organizations capable of meeting those needs; to help callers for whom services are unavailable by brainstorming alternative resources and, if necessary, to actively participate in linking the inquirer to needed services. UNIT: one client referral

Follow-up: To determine the outcome of a referral by contacting the client and/or the organization to which he/she was referred.

4. Area to be served: Monterey County

5. General requirements:  Establish a system of keeping up to date information on resources for seniors throughout the County.  Publication of a resource directory one time within the next four-year cycle  Establish a system for making referrals and following up contacts.

I. Service Name: LEGAL ASSISTANCE

1. Program Goal: To inform and educate older persons regarding their legal rights and entitlements.

2. Program Definition: Legal assistance consists of legal representation by a member of the California State Bar or by a non-attorney under the supervision of a member of the California State Bar. 3. Required Units of Service (M.I.S. definition):

Unit of Service definition(s):

Legal Services: To provide legal information, advice, counseling, administrative representation, and judicial representation to an individual or to a group by a member of the California State Bar or by a non-attorney under the supervision and control of a member of the California State Bar. Unit: One hour.

Legal Community Education/Training: to educate or train about senior’s rights, entitlements and benefits to older persons 60 years of age and older. Unit: One-hour presentation.

4. Area to be served: Monterey County

5. General requirements: Services shall include the following: 1. Legal representation and counseling to elders 60 years of age and older. 2. Give preference to serving individuals in greatest social and economic need, with particular attention to serving low-income minority individuals and older individuals residing in rural areas.

J. Service Name: MEDICATION MANAGEMENT

1. Program Goal: To improve consumers’ knowledge about medication management.

2. Program Definition: Educational services about medication use and management.

3. Required Units of Service (M.I.S. definition):

Medication Management: Education and Training—To provide educational services about medication use and management. UNIT: one session

4. Area to be Served: Monterey County

5. General requirements  Provide 6 community presentations on Medication Management  Research and development of curriculum and materials

K. Service Name: OMBUDSMAN FOR LONG TERM CARE

1. Program Goal: To assure proper care and treatment of older persons living in long term care institutions and prevent elder abuse in those institutions.

2. Definition: To provide community education, outreach, recruitment and training of volunteer ombudsman, and investigate allegations of complaints.

3. Required Units of Service (M.I.S. definition):

Unit of Service definitions:

Complaint/Investigation/Resolution: Activities related to receiving, analyzing, researching, observing, interviewing or verifying a complaint; activities related to intervention in a complaint on behalf of a client using skills and techniques such as advocacy, facilitation, conciliation, mediation, negotiation, representation, education, follow-up or referral. UNIT: One hour

Education/Training: To provide knowledge and skills training on long term care issues. UNIT: One hour

4. Area to be Served: Monterey County

5. General requirements:  Recruit, train and supervise volunteer ombudsman  Provide ongoing training to volunteers  Investigate complaints on behalf of persons in long term care institutions.  Make community presentations to educate the community on elder abuse prevention.

L. Service Name: OMBUDSMAN INITIATIVE

1. Program Goal: To provide advocacy services for residents living in long-term care facilities in Monterey County

2. Program Definition: To increase the number of Ombudsman volunteers working in skilled nursing facilities

Activities to be provided: Additional number of volunteers in Medi-Cal facilities: 2 or more Additional 36-hour certification training: 1 or more 4. Area to be Served: Monterey County

5. General requirements  Recruit volunteers from the community to increase the Ombudsman  Presence in long-term care facilities in Monterey County  Increase the number of volunteers in Medi-Cal facilities  Highten recruitment efforts in the Latino community to better serve this population  Provide 36-hour certification training at least once a year  Provide ongoing training, support and supervision to certified Ombudsman volunteers

M. Service Name: PERSONAL CARE/HOMEMAKER (IN-HOME SERVICES)

1. Program Goal: To assist frail older persons to remain in their own homes through assistance with activities of daily living.

2. Program Definition: In-Home Services consists of those services provided on an emergency, intermittent basis, which assist older persons to maintain independent living or provide respite for the primary caretaker. These services are: (1) bathing, dressing, feeding and personal hygiene; (2) cooking, shopping, or light housekeeping; and (3) heavy household tasks.

3. Required Units of Service (M.I.S.definition):

Unit of Service Definitions:

Personal Care: To assist clients with routine care of a personal nature such as bathing, dressing, personal appearance, feeding and general hygiene. UNIT: One Hour

Homemaker: To provide light routine maintenance of a household including dusting, vacuuming, laundry, meal preparation, or shopping UNIT: One Hour

4. Area to be served: Monterey County

5. General requirements

Part-time assistance to homebound elderly by competent, supervised homemakers. Services will include light housekeeping, personal care, meal preparation and other supportive activities. Provider(s) will serve as short-term provider(s) of homemaking services until a private-provider arrangement can be made. Page 11 of 16 ATTACHMENT F, RFP 9879, Area Agency on Aging

 Provision of long-term, ongoing care only to those clients who are identified by the AAA as in need of special personal care and/or who do not qualify for housekeeping/ homemaking services through the Title XX In-Home Supportive Services program.  Close coordination with social work staff of the Office for Aging and Adult Services/Area Agency on Aging  Services restricted to low income consumers as deemed by Social Security Administration/Medi-Cal income thresholds

N. Program Name: REHABILITATION AND EDUCATION

1. Program Goal: To improve the well being and physical, emotional functioning of older persons.

2. Program Definition: Rehabilitation, education and assistance to older persons who are 60+ years of age and are functionally impaired. Services are provided by health professionals and allied para-professionals.

3. Required Units of Service (M.I.S. Unit of Service number and definition): Education/training: To impart knowledge; provide information, or instruction on health topics in a group setting (excludes training of program staff or volunteers). Unit: One hour

Rehabilitation/Therapy: To provide treatment of a specific physical problem by a health professional or an allied health professional as permitted by law. Unit: One hour

4. Area to be Served: Monterey County

1. 5. General requirements:  Provide rehabilitation, education and assistance to older persons who are 60+ years of age and are functionally impaired.

O. Service Name: SENIOR CENTER MANAGEMENT/ACTIVITIES COORDINATION

Components:

1. Program Goal: To support and oversee the operation of a senior nutrition program in support of senior wellness and to foster socialization opportunities.

2. Program Definition: To encourage and increase attendance and participation at senior centers by providing center management services which include activities coordination and planning, and marketing.

3. Required units of service (M.I.S. definition)

Community Service/Senior Center Management (160): To support and oversee the operation of a senior nutrition program in support of senior wellness and to foster socialization opportunities. Unit: Hour

4. Area to be served: Monterey County

5. General requirements:  Complete registration and nutrition risk assessment for new lunch participants.  Plan and coordinate program activities

 Recruit, train and supervise volunteers  Maintain Serve-safe certification for food handling  Support and assist the functions of Senior Advisory Councils

P. Service Name: SENIOR EMPLOYMENT PROGRAM

1. Program Goal: To assist older adults in securing part-time/full-time employment.

2. Program Definition: Activities designed to maintain or obtain employment for older persons or to assist them in selecting and entering into a second career.

3. Required Units of Service (M.I.S. definition):

Unit of Service definitions:

Placement: To assist a person in securing part-time/full-time employment. UNIT: one placement

4. Area to be served: Monterey County

5. General requirements:  Marketing and promotion of the program.  Assessing the employment readiness skills of those seeking employment  Assessing employment needs of employers  Matching and following up on matches made

Q. Service Name: SENIOR FOOD BAG

1. Program Goal: To maintain or improve the well-being and nutritional health of older persons through the delivery of food bags (brown bags) to seniors throughout the county.

2. Program Definition: Community Services (Income Support/Material Aid) consisting of the distribution of food bags (brown bags) to older persons.

B. Unit of Service Definition(s)

Other Services – Community Service (Income Support/Material Aid) Unit: 1 individual contact

2. Area to be served: Monterey County

3. General requirements:

 Perform community service and information regarding availability of food services in the county  Distribute donated fruit, vegetables and other food products to seniors living in Monterey County  Recruit and maintain volunteers for the food distribution program;  Maintain an established system for collection of voluntary client contributions for services  Coordinate with other nutrition related service providers in Monterey County

R. Service Name: SENIOR PEER COUNSELING

1. Program Goal: To assist older adults experiencing some of the emotional problems that accompany the aging process.

2. Program Definition: Trained and supervised volunteer counselors offer emotional support and guidance to persons 60 years or older who may be facing the death of a spouse, the stress of an illness, isolation from family and friends, or other life transitions. The Program does not provide 24-hour crisis intervention, assessments for dementia or other brain impairments, or therapeutic counseling for addictions or major mental illnesses. 3. Required Units of Service (M.I.S. definition):

Unit of Service definitions:

Counseling: To provide emotional support to individuals in a group setting, or one-on-one, who are experiencing the loss of a loved one, the stress of an illness, isolation from family and friends and other life transitions. UNIT: One hour

Training: To provide training in individual counseling and support groups to volunteers who counsel seniors about issues they may be facing in the area of loss—of a spouse, of one’s health, or isolation from friends and family or other life transitions. UNIT: One hour

5. General requirements  Recruit, screen and train volunteer peer counselors  Recruit, screen and match those in need of counseling with peer counselors  Provide regular peer counseling training

S. Service Name: SHARED HOUSING

1. Program Goal: To assist older adults seeking shared housing.

4. Program Definition: Shared Housing Program will provide information to people seeking shared housing arrangements. The Program will include outreach, intake, assessment, information and the matching of persons seeking shared housing arrangements.

5. Required Units of Service (M.I.S. definition):

Information : To provide individuals with current information on opportunities and services available to the individuals within their communities, including information relating to housing opportunities. UNIT: one contact

Referral: To link individuals to the opportunities and services that are available for shared housing. UNIT: one client referral

4. Area to be served: Monterey County.

5. General requirements:  Recruiting seniors wishing to share their home with another individuals  Recruiting seniors wishing to share another individual’s home.  Developing opportunities for shared housing and publicize such opportunities throughout the community.

T. Service Name: VULNERABLE ELDER RIGHTS PROTECTION ACTIVITIES

1. Program Goal: To prevent elder abuse of older persons

2. Program Definition: To provide support of residents of long term care facilities through the provision of ombudsman activities. To develop and strengthen activities to prevent and treat elder abuse. To promote and support coordination activities among programs (e.g. multidisciplinary teams, interagency working groups and coalitions.)

3. Required Units of Service (M.I.S. definition):
