Under National Honor Society rules, this package cannot be considered as an application for admission; it is a data sheet, which will be used by the Faculty Council to assist in determining membership. The information you list will be taken into account along with your academic record and input from the faculty in reaching a final decision. You will turn in the following things: Signature page, Essay, Service Form (s), Leadership Form (s)

Please state, in typed essay form on a separate sheet of paper, why you would like to be a member of National Honor society. Include a prospective service project that you feel would benefit the school and/or the community. Keep your response to 300-500 words. Address the following points in your essay:  Your definition of National Honor Society  What role NHS plays at SHAPE HS  The major reason you wish to become a member  Your expectations of your role in NHS  What you feel is a reasonable time commitment to devote to NHS

If accepted for membership in the Emmanon Chapter of the National Honor Society, I will attest to the following: 1) I will arrive promptly and participate in the monthly meetings 2) I will participate in all NHS group service projects and mandatory activities 3) I will accumulate at least 10 hours of individual service per semester and turn in monthly documentation 4) I will maintain a GPA of at least 3.75 5) I will give written advance notice if for any reason I am not able to participate in a mandatory activity 6) I understand that failure in any of the above areas is grounds for removal

The completed application must be returned to NHS sponsor no later than 1530 hours Friday, 6 March in order to be used by the Faculty Council in making its decision. No exceptions to this deadline can be considered. The application must be complete, no partial application will be accepted.
