a) Maximum Order Threshold: $1 million per order. b) Payment Terms: Net 30 days. c) Minimum Assignment: 4 weeks. Travel costs will be negotiated with the facility level and cannot exceed the Federal Rates for travel and housing. Total for all costs cannot exceed the FSS Price. Government retains the right to terminate for convenience with no penalty. No cancellation and no placement fees awarded. d) Geographic coverage (service area): All 50 States and Washington DC e) Medical Liability Insurance: Amount of $1,000,000 per occurrence, $3,000,000 aggregate. f) Government Commercial Credit Card: Accepted for purchases up to the Micro Purchase amount. g) Qualifications of Professionals: See below (page 3) h) Background Checks: A web-based background investigation service is utilized to provide county, state, multi-state and national criminal investigations, sexual predator searches, most wanted and terrorist searches. Should the facility require a more extensive background investigation it will be negotiated at the task order level. i) Overtime Rate(s): 1.25 times the hourly base rate j) Shift Differentials: None k) On-Call Rate(s): On- Call $6.00/hour. A pager or cell phone is required to be worn by provider. Provider must remain within 30 minutes radius or the agreed to distance of the facility. l) Call Back Rate(s): The regular hourly rate will apply once the provider begins work onsite. OT will be billed and paid for all hours worked if OT is being incurred, not inclusive of on-call time. m) Holiday Rate(s): All holidays recognized by the federal government. OT will be billed and paid for all hours worked if OT is being incurred. n) Work week: Sunday to Saturday

All JCAHO requirements shall be met at time of hire and maintained appropriately.
