Antibiotic Stewardship in Nursing Homes Collaborative
Overview: The U.S. Centers for Medicare & Medicaid Services has mandated that by November 28, 2017, Medicare-certified nursing homes must have implemented an antibiotic stewardship program. LeadingAge Minnesota is partnering with The University of North Carolina’s (UNC’s) Program on Aging, Disability & Long-Term Care to facilitate and support an Antibiotic Stewardship Collaborative for LeadingAge Minnesota members building on a successful UNC grant project which utilized resources developed specifically for antibiotic stewardship in nursing homes by UNC. Collaborative Goals:
Optimize appropriate use of antibiotics in LeadingAge Minnesota member nursing homes leading to a decrease in potential undesirable outcomes of inappropriate antibiotic use, such as C. difficile infection, preventable hospitalizations, and antimicrobial resistance.
Meet the CMS Requirements of Participation for establishing an Antibiotic Stewardship program by November 2017. Collaborative Structure:
Participation Agreement - Complete an Antibiotic Stewardship participation form signed by leadership and designating a champion to serve as the lead for the collaborative.
Convene Team – Establish an Antibiotic Stewardship team (i.e., DON, infection control nurse, medical director, others as desired).
Kick-Off – Participate in a kick-off virtual session with UNC faculty providing content expertise.
Data Collection – Collect standardized data on infections diagnosed, antibiotics prescribed and outcomes such as hospitalizations with analysis and feedback reports provided back to participants by UNC.
Monthly Champion Check-Ins – Meet virtually with the UNC faculty monthly to review data and discuss barriers and successes.
Quarterly Team Check-Ins – Participate in conference calls with UNC faculty and Collaborative colleagues to learn and discuss new content and address implementation barriers. Continued Improvement & Sustainment – Following the initial 6-month implementation phase, continue virtual meetings to reinforce sustainment, address implementation barriers, and share new innovations.
Resources - A comprehensive set of training resources are available to participants designed specifically to support nursing homes in establishing an effective antibiotic stewardship program. Resources include:
o Infection Tracking Spreadsheets
o Training Materials for Nursing Staff
o Training Materials for Physicians, Nurse Practitioners, and Physician Assistants
o Instructional Materials for Residents and Families
o Manual on How to Implement an Antibiotic Stewardship Program

Collaborative Maintenance Phase: (includes quarterly supporting webinars): November 2017 – May 2018 Costs:
$300 (plus shipping for materials) per participating organization for 1-year all-inclusive program (materials, training, technical assistance) o For larger organizations, there is an opportunity to purchase a larger package of materials for $450 (plus shipping for materials).
