<p> Antibiotic Stewardship in Nursing Homes Collaborative</p><p>Overview: The U.S. Centers for Medicare & Medicaid Services has mandated that by November 28, 2017, Medicare-certified nursing homes must have implemented an antibiotic stewardship program. LeadingAge Minnesota is partnering with The University of North Carolina’s (UNC’s) Program on Aging, Disability & Long-Term Care to facilitate and support an Antibiotic Stewardship Collaborative for LeadingAge Minnesota members building on a successful UNC grant project which utilized resources developed specifically for antibiotic stewardship in nursing homes by UNC. Collaborative Goals: </p><p> Optimize appropriate use of antibiotics in LeadingAge Minnesota member nursing homes leading to a decrease in potential undesirable outcomes of inappropriate antibiotic use, such as C. difficile infection, preventable hospitalizations, and antimicrobial resistance. </p><p> Meet the CMS Requirements of Participation for establishing an Antibiotic Stewardship program by November 2017. Collaborative Structure:</p><p> Participation Agreement - Complete an Antibiotic Stewardship participation form signed by leadership and designating a champion to serve as the lead for the collaborative. </p><p> Convene Team – Establish an Antibiotic Stewardship team (i.e., DON, infection control nurse, medical director, others as desired).</p><p> Kick-Off – Participate in a kick-off virtual session with UNC faculty providing content expertise.</p><p> Data Collection – Collect standardized data on infections diagnosed, antibiotics prescribed and outcomes such as hospitalizations with analysis and feedback reports provided back to participants by UNC. </p><p> Monthly Champion Check-Ins – Meet virtually with the UNC faculty monthly to review data and discuss barriers and successes. </p><p> Quarterly Team Check-Ins – Participate in conference calls with UNC faculty and Collaborative colleagues to learn and discuss new content and address implementation barriers. Continued Improvement & Sustainment – Following the initial 6-month implementation phase, continue virtual meetings to reinforce sustainment, address implementation barriers, and share new innovations. </p><p> Resources - A comprehensive set of training resources are available to participants designed specifically to support nursing homes in establishing an effective antibiotic stewardship program. Resources include:</p><p> o Infection Tracking Spreadsheets </p><p> o Training Materials for Nursing Staff</p><p> o Training Materials for Physicians, Nurse Practitioners, and Physician Assistants</p><p> o Instructional Materials for Residents and Families</p><p> o Manual on How to Implement an Antibiotic Stewardship Program</p><p> Individual Technical Support – Up to 2 hours of individual technical support is available from UNC faculty.
